资癸女贞胶囊对子宫内膜薄性不育症妊娠结局影响的研究

Effects of Zi Gui Nv Zhen capsule on pregnancy outcome in infertility due to thin endometrium

目的观察资癸女贞胶囊对子宫内膜薄性不育症妊娠结局的影响。方法观察165例子宫内膜过薄导致不育症的患者,采用随机数字表法分为观察组80例和对照组85例。对照组单纯应用雌二醇治疗,在对照组治疗的基础上观察组加服资癸女贞胶囊,监测排卵后子宫内膜螺旋动脉血流搏动指数及阻力指数,子宫内膜厚度,黄体期血清孕酮及雌二醇质量浓度,追访妊娠及活产情况。结果观察组较对照组子宫内膜厚度明显增加,A型内膜数量增加(P<0.05);观察组排卵日子宫动脉血流搏动指数及阻力指数均明显低于对照组(P<0.05);治疗3个周期后,观察组妊娠率明显高于对照组(P<0.05);随访观察组妊娠率明显高于对照组(P<0.05);活产率两组差异没有统计学意义(P>0.05)。结论应用资癸女贞胶囊联合雌二醇治疗子宫内膜薄性不育症,资癸女贞胶囊可通过降低子宫动脉血流阻力增加子宫动脉的血流量,通过增加雌激素受体表达,使雌激素能更好的发挥作用,进而改善子宫内膜厚度及类型,提高子宫内膜容受性,中药复方制剂通过调节全身神经内分泌系统,改善卵巢功能,提高卵子质量,从而提高临床妊娠率。

Objective To observe the effects of Zi Gui Nv Zhen capsule on pregnancy outcome in infertility due to thin endometrium . Methods To observe one hundred sixty-five infertility patients with thin endometrium .These patients were divided into two groups by random number table method .The observation group had 80 cases, control group 85 cases.The control group was treated with estradiol valerate alone .Besides the treatment given to the control group , patients of observation group were given Zi Gui Nv Zhen capsule for treatment.Endometrial spiral artery blood flow pulsation index （PI） and endometrial spiral artery blood flow resistance index （RI）, endometrium thickness , luteal phase serum progesterone and estradiol of after ovulation were observed and the patients were followed up for pregnancy and live birth rates .Results The thickenness of observation group type A endometrium of patients increased significantly （P＆lt;0.05） as compared with the control group.Uterus artery RI and PI in ovulation day of patients in the observation group was significantly lower than that of control group （P＆lt;0.05）.After three cycles the observation group had significantly higher pregnancy rate , follow-up found that the pregnancy rate in the observation group was significantly higher than that of the control group （P＆lt;0.05）; live birth rate in two groups had no significant statistical difference .Conclusion Application of Zi Gui Nv Zhen capsule combined with estradiol valerate therapy in thin endometrium of infertile patients could reduce uterine artery resistance , increase uterine artery blood flow, increase the expression of estrogen receptor , so estrogen can better play its role , thereby improved endometrial thickness and type , to improve endometrial receptivity traditional Chinese medicine might regulate systemic neuroendocrine system , improve ovarian function , improve ovum quality , thereby improve clinical pregnancy rate .
