摘要
目的探讨肝窦阻塞综合征(hepatic sinusoidal obstructive syndrome,HSOS)的CT特点,以提高诊断准确率。方法回顾性分析14例经临床或病理诊断肝窦阻塞综合征CT表现。结果 14例均表现为肝肿胀伴肝实质密度不均匀减低,其中以第二肝门为中心沿三支肝静脉周围"放射状"稍高密度9例(9/10)。增强动脉期肝实质斑片状强化4例,未见明显强化10例;门静期地图样强化10例(10/14),其中以第二肝门为中心沿三支肝静脉周围"放射状"强化9例(9/10),斑片状强化4例;延迟期(包括"放射状"强化)强化范围扩大,但仍不均匀。肝动脉稍增粗7例,门静脉增宽6例,肝段下腔静脉受压13例,门静脉、下腔静脉周围水肿5例,肝静脉均显示不清。结论基于门脉期肝实质以第二肝门为中心沿三支肝静脉周围"放射状"地图样强化,有助于HSOS的准确诊断。
Objective To study the CT features of hepatic sinusoidal obstruction syndrome (HSOS) in order to improve the diagnostic accuracy. Methods The CT findings of 14 cases of the hepatic sinusoidal obstruction syndrome by clinical or pathological diagnosis were retrospectively analysed. Results All 14 cases showed liver swelling and uneven decrease of liver parenchyma density, in which 9 cases displayed "radial shape" slightly high density centered on the second hepatic door along the around of three hepatic vein. On CT contrast, the liver parenchyma showed patchy enhancement in 4 cases, no obvious enhancement was found in 10 cases on the arterial phase;"map pattern" enhancement was found in the 10 cases (10/14), of which slightly high density centered on the second hepatic door along the around of three hepatic vein appeared "radial shape" enhancement in 9 cases (9/10), and patchy enhancement appeared in 4 cases that displayed on the portal ve- nous phase~ the scope of enhancement (including the "radial shape" enhancement) expanded on the delayed phase, but still uneven. The slightly enlargement of hepatic artery was found in 7 cases, the broadening of portal vein in 6 cases, the pres- sure of inferior vena cava in 13 cases, the edema of portal vein and inferior vena cava surrounding in 5 cases. The unclear hepatic vein were all shown. Conclusion Based on "radial shape" that centered on the second hepatic door along the a- round of three hepatic vein of "map pattern" enhancement on the portal phase will help HSOS accurate diagnosis.
