摘要
目的:研究兔脊柱融合术后邻近椎间盘软骨终板组织结构和形态学改变。方法:54只体重相当的12月龄日本大耳兔随机等分为2组,一组为实验组,使用椎弓根螺钉和钢丝内固定的方法融合L 5/6节段,另一组作为对照组,仅做相同部位皮肤切口后缝合。分别于术后3、6、9mo每组取8只动物处死并取出L 2/3、L 4/5及L 6/7软骨终板进行HE染色及Markin评分。结果:融合术后3、6及9mo时实验组L 4/5、L 6/7(邻近节段)软骨终板Mankin评分较对照组高,差异有统计学意义(P<0.05)。实验组L 4/5(融合平面以上邻近节段)与L6/7(融合平面以下邻近节段)软骨终板Mankin评分6、9mo时较对照组高,差异有统计学意义(P<0.05)。结论:脊柱融合内固定术后邻近节段软骨终板较未融合固定者易发生退变,并且融合平面以上邻近节段较融合平面以下邻近节段更易发生退变。
Objective:To study the structure and morphous of the adjacent cartilage endplate in rabbits after spinal fusion. Methods:Totally 54 twelve-month old Japanese rabbits were randomly divided into two groups. Each group included 27 rabbits. One group was experimental group,another one was control group. L 5/6 of all the experimental rabbits was surgery fused using screws and steel. The control group model was incised only the skin. 8 rabbits of each group were killed at three,six and nine months. After death,L 2/3,L 4/5 and L 6/7 spines were harvested. All the exemplar hematoxylin and eosin stain were performed. Results:Markin score of adjacent segment ( L 4/5 and L 6/7 ) in experimental group were higher than that in control group ( P〈0 . 05 ) . The differences of Markin score between L 4/5(the adjacent segment above the fusion level)and L 6/7(the adjacent segment below the fusion level)in the experimental group were statistically significant at 6month and 9month after spinal fusion(P〈0. 05). Conclusion:The cartilage endplate of adjacent segment were easier to degenerate after spinal fusion,and adjacent segment above the fusion level was easier to degenerate than that below the fusion level.
