摘要
目的探讨幽门螺杆菌(Hp)感染对患者血脂的影响,为临床诊治提供依据。方法将2012年1月-2012年6月90例Hp感染并血脂异常患者,随机分为根除组和降脂组各45例,根除组采用奥美拉唑20mg+阿莫西林0.5g+甲硝唑0.4g,每天2次,治疗两周后序贯奥美拉唑2周,疗程为4周,疗程结束4周后复查14 C-尿素呼气试验和血脂检测;降脂组采用阿托伐他汀20mg,每天1次,疗程为8周,疗程结束时进行血脂检测,对其结果进行统计分析。结果根除组Hp根除率为77.78%;根除组的总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)较根除前分别下降1.17mmol/L、0.67mmol/L、0.62mmol/L,差异均有统计学意义(P<0.05),降脂组TC、TG、LDL-C分别下降1.09mmol/L、0.62mmol/L、0.74mmol/L,差异有统计学意义(P<0.05);根除组的TC、TG、LDL-C降脂有效率分别为75.56%、64.44%、62.22%,降脂组分别为77.78%、57.78%、55.56%;而两组HDL-L升高有效率分别为51.11%和53.33%,根除组具有改善血脂代谢作用。结论根除Hp感染能有效地降低Hp感染患者的血脂水平,可作为临床病因治疗。
OBJECTIVE To investigate the influence of Helicobacter pylori infection to the blood-lirid ,so as to provide the evidences for clinical treatment .METHODS Totally 90 cases of abnormal blood-fat with positive HP were selected and were divided into 2 groups randomly .The radical cure group was given 20mg omeprazole and 0 .5 g amoxicillin and 0 .4g arilin two times daily for 2 weeks ,then omeprazole were given for 2 weeks .14C-urea exhalation test and blood-lirid test were carried out .The control group was given 20mg atorvastatin one time daily for 8 weeks ,and then the blood-lirid was tested after the treatment . The results were analyzed statistically . RESULTS The radical cure rate of the radical cure group was 77 .78% ;total cholesterol(TC) ,triglyceride(TG) , low density lipoprotein cholesterol (LDL-C ) were reduced by 1 .17 mmol/L ,0 .67 mmol/L ,0 .62 mmol/L , respectively ,the differences were significant ( P 〈 0 .05 ) , TC , TG , LDL-C were reduced 1 .09 mmol/L , 0 .62 mmol/L ,0 .74 mmol/L in control group ,the differences were significant (P〈 0 .05) .The reduced rate of TC ,TG ,LDL-C in radical cure group were 75 .56% ,64 .44% ,62 .22% ,while 77 .78% ,57 .78% ,55 .56% in control group .The HDL-L raised effective rate were 51 .11% and 53 .33% respectively .There was improving effect of blood-fat metabolism in radical cure group .CONCLUSION Hp radical cured can reduce effectively blood-lirid level in patients with positive Hp of abnormal blood-lirid .And it can be used as a clinical etiology treatment .
