腹腔镜全子宫切除术患者围术期血清中粘连及应激相关指标的变化研究

Study on the serum adhesion and stress related indexes of patients with total laparoscopic hysterectomy during perioperative period

目的研究分析腹腔镜全子宫切除术患者围术期血清粘连及应激相关指标的变化情况。方法选取2012年4月~2014年1月于该院进行全子宫切除术治疗的90例患者为研究对象,根据手术方式分为对照组（开腹手术组）45例和观察组（腹腔镜手术组）45例,然后将两组患者术前2 d及术后3、5和7 d的血清粘连及应激相关指标进行统计与比较。结果观察组术后3、5和7 d的血清粘连及应激相关指标均明显低于对照组,且观察组术后3 d的应激相关指标明显高于其他时间,术后5 d的粘连相关指标高于其他时间,均P〈0.05,差异均有显著性。结论腹腔镜全子宫切除术患者围术期血清粘连及应激相关指标的变化明显,但是其波动均小于开腹手术,更有助于患者术后的康复。

[ Objective ] To investigate and analyze the serum adhesion and stress related indexes of patients with total laparoscopic hysterectomy during the perioperative period. [ Methods ] 90 patients with total hysterectomy in our hospital from April 2012 to January 2014 were selected in our study, and 90 cases were randomly divided into control group （open operation group） 45 cases and observation group （laparoseopie operation group） 45 cases according to the operation types, then the serum adhesion and stress related indexes of two groups were analyzed and compared at day 2 before the operation and at days 3,5,7 after the operation . [ Results ] The serum adhesion and stress related indexes of observation group at days 3,5,7 after the operation were all obviously lower than those in control group, and the stress related indexes of observation group at third day after the operation were all obviously higher than those at other times, and the adhesion related indexes at fifth day after the operation were higher than those at other times, all P 〈0.05, there were all significant differences. [ Conclusion ] The change of serum adhesion and stress related indexes of patients with total laparoscopic hysterectomy during the perioperative period are obvious, while the fluctuation is all smaller than those of patients with open operation, so it is more helpful for the postoperative rehabilitation of patients.
