Ⅱ型糖尿病患者合并高血压的临床研究

Clinical study on hypertension in patients with type 2 diabetes mellitus

目的探讨Ⅱ型糖尿病合并高血压患者的血管病变情况。方法分析住院治疗的310例患者,将其分为糖尿病组(DM)82例、糖尿病合并高血压组(DM-HT)92例、高血压组(HT)74例、非糖尿病非高血压组(N-DM-N-HT)62例。对四组患者的血糖、血脂、血压、病程进行分析,并统计四种类型血管疾病的发生情况。结果 HT组、DM-HT组、DM组的BMI显著高于N-DM-N-HT组(P<0.05)。HT组、DM-HT组的血糖显著高于N-DM-N-HT组(P<0.05)。HT组、DM-HT组、DM组的TC、TG、LDLC显著高于N-DM-N-HT组(P<0.05)。DM-HT组的病程显著长于HT组和DM组(P<0.05)。DM组及HT组大、小血管并发症的发病率明显高于N-DM-N-HT组(P<0.01)。并且DM-HT组最高56.6%;DM主要引起小血管病变47.5%,HT主要引起大血管病变16.2%;且与N-DM-N-HT组比较差异具有显著性(P<0.01);DM、HT、DM-HT组与大小血管并发症发生密切相关(P<0.01)。结论 DM、HT、DM-HT是心、脑血管病的独立危险因素,与血管并发症密切相关。

Objective To explore the type 2 diabetic patients with vascular lesions in patients with hypertension.Methods 310 patients were divided into diabetes mellitus group （DM,82 cases）,92 cases of diabetes complicated with hypertension （DM-HT group）,74 cases of hypertension group （HT）,and 74 cases of non-diabetic non hypertensive group （N-DM-N-HT）.Blood glucose,blood lipid,blood pressure,duration of four groups of patients was analyzed.Results HT group,DM-HT group,DM group,BMI was significantly higher than that in group N-DM-N-HT （P〈0.05）.Group HT,group DM-HT blood glucose was higher than that in N-DM-N-HT group （P〈0.05）.HT group,DM-HT group,DM group,TC,TG,LDLC were significantly higher than that of group N-DM-N-HT （P〈0.05）.DM-HT group was significantly longer than those of group HT and group DM （P〈0.05）.the incidence of microvascular complications in DM group and HT group was significantly higher than that in N-DM-N-HT group （P〈0.01）.And DM-HT was the highest 56.6%; DM was mainly caused by small vessel disease; the rate was 47.5%,HT mainly caused by big vascular l disease; the rate was 16.2%; and compared with the N-DM-N-HT group the difference was significant（P〈0.01）; DM,HT,DM-HT group and the size of vascular complications were closely related （P〈0.01）.Conclusion DM,HT and DM-HT are independent risk factor for heart and cerebrovascular disease.
