1例糖尿病肾病V期患者的药学监护

1 Case Report of Pharmaceutical Care for Diabetic Nephropathy Patients with Stage V

目的探讨临床药师参与糖尿病肾病患者用药治疗的实践与体会。方法介绍临床药师参与1例糖尿病肾病的治疗过程,根据患者病理生理情况,结合相关的药物学知识和临床治疗指南,与医师共同制订个体化用药方案。结果临床药师发挥了自身药学专长,提出合理的药物治疗建议,使患者各项体征好转。结论临床药师通过药学监护可避免药物不良反应的发生,提高药物的治疗水平。

Objective To discuss the practice and experience of clinical pharmacists participating in the clinical consultation of diabetic nephropathy patients with stage V. Methods Clinical pharmacist actively participated in the treatment of a patient with diabetic nephropathy. According to patients＇ pathophysiology and clinical practice guidelines,clinical pharmacist cooperated with physicians to make individual regimens. According to patients in pathophysiogical situation,combined with pharmaceutical knowledge and clinical practice guidelines,the clinical pharmacists cooperated with physician to make individual regimens. Results The clinical pharmacists can exploit their pharmaceutical expertise and make rational recommendations on drug therapy in order to improve the symptoms of patient. Conclusion The pharmaceutical care provided by clinical pharmacists can avoid the occurrence of adverse drug reactions and improve the level of drug therapy.
