血糖波动对2型DM周围神经病变患者血浆同型半胱氨酸水平的影响

摘要 目的:观察2型糖尿病患者在血糖波动状态下血浆同型半胱氨酸水平的变化,探讨血糖波动对糖尿病周围神经病变患者同型半胱氨酸水平的影响。方法:选取DPN患者75名分为血糖稳定组( DPN+1组)及血糖波动组( DPN+2组),30名无DPN患者( DPN-组),以及健康对照组( NC组),空腹检测血浆同型半胱氨酸(Hcy)、血糖(FPG)、餐后2小时血糖(2hPG)、糖化血红蛋白(HbA1c)等。结果:Hcy水平DPN+组及DPN-组明显高于NC组(P<0.01),DPN+2组较DPN+1组升高(P<0.05)。结论:提示Hcy水平与血糖相关,且与血糖波动密切相关,Hcy水平升高是DPN的危险因素。 Objective:To observe the level of homocysteinemia in type 2 diabetes mellitus patients with peripheral neuropathy under blood glucose fluctuantion, and investigate the effect of blood glucose fluctuantion to the level of homocysteinemia in patients with pe-ripheral neuropathy. Methods:Selected seventy-five type 2 diabetes mellitus patients with peripheral neuropathy to divide into steady-blood glucose-group ( DPN+1 group) and fluctuant-blood glucose-group( DPN+2 group), thirty type 2 diabetes mellitus pa-tients without peripheral neuropathy, and control group. The level of homocysteinemia, fasting plasma glucose, 2h plasma glucose, Hb A1c in plasma was measured.Results:The levels of homocysteinemia was more higher in DPN+1 group and DPN+2 group than in con-trol(P〈0.01), and was higher in DPN+2 group than in DPN+1 group(P〈0.05).Conclusions: The level of homocysteinemia is closely related to plasma glucose, especially to blood glucose fluctuantion. High level of homocysteinemia is a risk factor for diabetic neuropathy in humans.
