早期综合康复训练护理联合常规药物治疗脑卒中偏瘫随机平行对照研究

Hemiplegic Stroke Parallel Randomized Controlled Study of Early Comprehensive Rehabilitation Therapy combined with Routine Care

[目的]观察早期综合康复训练护理联合常规药物治疗脑卒中偏瘫疗效。[方法]使用随机平行对照方法,将60例住院患者按住院病志号简单随机分为两组,脑血管病常规药物治疗和护理。对照组28例急性期制动、卧位,住院5∽7d病情稳定后康复训练,1次/d。治疗组32例早期综合康复训练,脑梗死入院当天开始,脑出血48∽72h病情稳定即可进行;康复训练：一对一形式,一名护士负责一名病人;被动运动、主动运动、活动能力练习、针灸（头针、体针）、推拿按摩、情志护理。连续治疗护理1周为1疗程。观测临床症状、运动功能（Fugl-Meyer评分）,ADL（Barthel指数）、不良反应。连续治疗2疗程,判定疗效。[结果]Barthel指数两组均有改善（P〈0.01,P〈0.05）,治疗组改善优于对照组（P〈0.01）;FuglMeyer评分两组均有改善（P〈0.01,P〈0.05）,治疗组改善优于对照组（P〈0.01）。[结论]早期综合康复训练护理联合常规药物治疗脑卒中,疗效满意,无副作用,值得推广。

[ Objective ] To observe the Efficacy of stroke hemiplegia patients who were cured by early comprehensive rehabilitation therapy combined with routine care. [ Methods ] Through random parallel control method, 60 hemiplegia Patients in our hospital were randomly divided into two groups by hosital medical record number. The control group of 28 patients were given braken and supine in acute phase, take rehabilitation training from 5 to 7 days when the condition is stable ; one times a day. The treatment group of 32 patients were given early comprehensive rehabilitation training , The infarction patients were given the training when the disease was beginning, the cerebral hemorrhage patients can begin when the condition is stable from 48 to72 hours.Rehabilitation training：one for one , a nurse in charge of a patient ; passive movement , active movement, activity exercise , acupuncture （ scalp and body acupuncture ）, massage , emotional care , a cycle for continuous treatment of one week. We observe Motor function （ Fugl-Meyer score ）, ADL （ Barthel Index ）, adverse reactions and determine efficacy after 2 cycles. ~ Results ] Barthel index is improvement in both groups （ P 〈0.01, P〈0.05 ）, the improvement in the treatment group is better than the control group （ P〈0.01 ） ;Fugl- Meyer score is also improvement in both groups （P〈0.01, P〈0.05 ）, the improvement in the treatment group is better than the control group （ P〈0.01 ） . [ Conclusion ] The early implementation of comprehensive rehabilitation nursing measures can significantly improve the patient ＇ s motor function and ADL ability, no side effects , it is worth promoting.
