Immunological treatment of gastric precancerous lesions

胃癌癌前病变(precancerous lesions of gastric cancer,PLGC)主要表现为胃黏膜上皮腺体萎缩、上皮纤维化和胃黏膜上皮肠化生,无论是何种病理表现,其都可以表现为正常的组织异常化。现在研究认为造成胃黏膜萎缩的主要是幽门螺旋杆菌的感染和自身免疫系统的作用。幽门螺旋杆菌造成的胃黏膜损伤主要起源于幽门,逐渐向胃底扩展[1];但是由免疫系统引发的损伤却主要起始于胃底,逐渐向幽门进发。同时这两者之间可以相辅相成协同作用,造成PLGC病理状态的不断恶化。既往关注幽门螺旋杆菌的研究较为多见,在此篇综述中,笔者将免疫相关因素与PLGC发生发展的关系做一详细的论述。

The main features of PLGC are gastric mucosa atrophy , fibrotic material and intestinal metaplasia , the appropriate in the gastric mucosa is progressively reduced and replaced with nonapprppriate tissues.Nowadays scholars consider the Helicobater pylori infection and autoimmune gastritis are the two main causes.The former index induces PLGC changes starting from the antrum and incisura angularis extending to the corpus.The latter shows the main injure restricted to the corpus extending to antrum .Both H.pylori-associated and autoimmune are linked to an increased PLGC risk .In the previous times , H.pylori is focused more .In this review, we will discuss the relationship between autoimmune and PLGC in detail .
