创可贴纱块双压法在经桡动脉介入后穿刺部位加压止血的效果观察

Observation of band-aid joint Gauze piece of Dual pressure method in compression hemostasis for puncture point after transradial coronary intervention

目的探讨创可贴纱块双压法在经桡动脉行冠状动脉介入治疗后穿刺局部创口加压止血的效果。方法选择心血管内科100例经桡动脉行冠心病介入治疗的患者,按住院先后分为对照组和观察组,各50例。对照组患者在经桡动脉行冠心病介入拔鞘管后穿刺部位采用桡动脉介入穿刺部位止血器压迫止血的方法,观察组患者采用创可贴联合纱块双压法进行加压止血的方法。观察比较两组患者在按压止血时间、渗血、淤血、肢端肿胀等情况。结果两组平均持续加压止血时间观察组明显短于对照组,在渗血、淤血、肢端肿胀方面的发生率明显低于对照组,两组比较有明显差异性(P<0.05),具有统计学意义。结论采用创可贴联合纱块双压法应用于穿刺拔鞘后局部创口的压迫止血时间明显缩短,止血效果好,对静脉回流受阻小,能降低拔鞘后的并发症,提高患者的舒适度,减轻病人的经济负担,能代替桡动脉介入穿刺部位止血器的应用,且能适应于各种穿刺创口的加压止血,能增加社会效益和经济效益。

Objective To explore the effect of compression hemostasis for local puncture point after percutaneous radial artery approach for coronary intervention .Methods One hundred patients who required coronary intervention in cardiovascular department were divided into control group and observation group （ n=50） according to they were in hospital in time sequence .In the control group , patients who accepted transradial coronary intervention were adopted arteries radials pneumatic tourniquet to hemostasis after pulling out sheath ;while the observation group were adopted band-aid joint gauze piece of dual pressure method to compression hemostasis.The hemostatic time , the condition of exudation , congestion and hematoma were observed in each group.Results Duration of compression hemostasis in the observation group was significantly shorter than that in the control group .The incidence rate of exudation , congestion , hematoma and limbs swelling were significantly lower than those in the control group （all P〈0.01）.Conclusions Band-aid joint gauze piece of dual pressure method is applied to local wound after pulling out sheath .The dual pressure method gives an effective hemostasis , can significantly reduce ligature time , avoids obstruction of venous reflux , and reduces the incidence of complications after pulling out sheath .It can take the place of disposable arteries radials pneumatic tourniquet , and suitable for all kinds of direct pressure of puncture wounds .The application of band-aid joint gauze piece of dual pressure method allows for improved the comfort of patients , lighten patients economic burden , increased the economic benefits and made societal benefits .
