摘要
目的探讨瘦素及其受体在再生障碍性贫血(AA)病人血浆和骨髓中的表达及其意义。方法应用酶联免疫吸附试验(ELISA)方法检测38例AA病人(其中重型者8例、非重型者30例)及22例健康体检者血浆中瘦素及可溶性瘦素受体(sLR)的水平;应用PV-6000二步法检测32例AA病人(其中重型者14例、非重型者18例)及17例对照组骨髓组织中瘦素及其受体的表达。结果 AA病人血浆瘦素水平为(14.16±4.50)μg/L,显著高于对照组的(10.87±3.21)μg/L(t=3.008,P<0.01);AA病人血浆sLR水平为(16.85±4.89)μg/L,显著低于对照组的(20.67±6.18)μg(t=2.641,P<0.05)。AA病人骨髓组织中瘦素表达水平为0.192±0.030,显著高于对照组的0.168±0.024(t=2.808,P<0.01);AA病人骨髓组织中瘦素受体表达水平为0.198±0.041,显著低于对照组的0.235±0.054(t=2.704,P<0.05)。重型AA病人血浆及骨髓中瘦素水平高于非重型者,瘦素受体水平低于非重型者,但差异无显著性(P>0.05)。结论 AA病人血浆及骨髓中瘦素水平升高,而受体水平下降;瘦素在AA的发生发展中有失利用现象存在。
Objective To investigate the expressions of leptin and its receptor in plasma and bone marrow in patients with aplastic anemia(AA). Methods The plasma levels of leptin and its plasma leptin receptor(pLR)were detected in 38 AA patients-severe AA eight patients,non-severe AA 30patients-and 22 healthy individuals with enzyme-linked immunosorbent assay(ELISA),and immunohistochemistry PV-6000 was applied to detect the expressions of leptin and its receptor in bone marrow of 32 AA patients-severe AA 14 patients,non-severe AA 18patients-and 17 normal controls. Results The plasma leptin levels in AA patients were higher than the healthy controls,(14.16±4.50)μg/L versus(10.87±3.21)μg/L(t=3.008,P〈0.01),and the pLR levels in the patients were lower than the healthy controls,(16.85±4.89)μg/L versus(20.67±6.18)μg/L(t=2.641,P〈0.05).The expression levels of leptin in bone marrow in the patients were 0.192±0.030,much higher than 0.168±0.024 in the control group(t=2.808,P〈0.01);and that of pLR were 0.198±0.041,much lower than 0.235±0.054 in the normal control group(t=2.704,P〈0.05).The levels of leptin in both plasma and bone marrow in severe AA patients were higher than that in non-severe AA,and that of leptin receptor were lower,but the differences were not statistically significant(P〉0.05). Conclusion The levels of leptin in plasma and bone marrow in patients with aplastic anemia rise,and that of leptin receptor decline.Achrestic condition of leptin exists in the occurrence and development of aplastic anemia.
