牙列缺损修复治疗对口腔健康相关生活质量的影响

THE INFLUENCE OF PROSTHODONTIC TREATMENT ON ORAL HEALTH-RELATED QUALITY OF LIFE

目的探讨牙列缺损病人修复治疗后口腔健康相关生活质量（OHRQoL）的改变,为临床治疗方案的选择和治疗效果的评价提供依据。方法临床随机收集门诊就诊并进行修复治疗的牙列缺损病人95例,75例行可摘局部义齿（RDP）修复,20例行固定义齿（FDP）修复。采用口腔健康影响程度量表（OHIP-14）中文版分别于修复前和修复后6周测量其OHRQoL,分析修复治疗对病人OHRQoL的影响。结果两组病人义齿修复后OHIP-14总分明显降低,差异有统计学意义（Z=5.165、3.826,P〈0.01）。RDP修复后,病人经历负面影响的比例明显降低,有7个条目差异有统计学意义（χ2=6.313~26.552,P〈0.05）;而FDP修复后仅有2个条目差异有统计学意义（χ2=4.902、9.176,P〈0.05）。结论两种修复方式均可提高牙列缺损病人的OHRQoL;相比FDP修复,RDP修复后的OHRQoL提升幅度更大。

Objective To investigate the changes of oral health related quality of life （OHRQoL） in patients with defect of dentitionv （DOD） after prosthodontic treatment and provide a basis for clinical selection of treatment protocols and assessment of therapeutic effectiveness. Methods Ninety-five patients with DOD underwent prosthodontic treatment were enrolled. Of whom, 75 patients received removable partial denture （RDP）, and 20 received fixed denture （FD）. All of them were assessed for their OHRQoL by using Chinese-version OHIP 14 before and six weeks after the treatment. Results The OHIP-14CN total score of patients in the two groups decreased significantly after treatment （Z=5.165,3.826;P 〈0.01）. After RDP treatment, the percen- tage of patients who experienced negative influence dramatically declined, of which, the differences of seven items were statistically significant （x2= 6.313 26.552,P〈0.05）, for the FDgroup, there were only two items with statistical significance （X2=4.902, 9.176;P〈0.05）. Conclusion Both the removable partial denture and fixed denture treatments improve OHRQoL of patients with defect of dentition. Compared with fixed denture, the removable one improves greater OHRQoL.
