妇产科医院常见静脉用药不合理分析

Analysis of irrational intravenous medication in obstetrics and gynecology hospital

目的分析我院静脉用药医嘱审核中常见的不合理用药问题,为临床安全、有效用药提供参考。方法对我院静脉用药调配中心2011年1月至2012年12月期间审核记录的126条不合理静脉用药医嘱进行统计和分析。结果不合理医嘱包括溶媒选择不合理、用法用量不合理、医嘱输入错误、配伍禁忌等。溶媒选择不合理占比最高(42.1%),主要集中在铂类抗肿瘤药物。结论药师审核用药医嘱,发现问题加以干预,可减少不合理用药发生,确保患者用药安全。

AIM To analyze the irrational intravenous medication during medication audit in our hospital for reference of safe and effective drug use in the clinic. METHODS A total of 126 cases of irrational intravenous medication orders of pharmacy intravenous admixture service audited in our hospital from January 2011 to December 2012 were analyzed statistically. RESULTS Irrational medication orders included irrational selection of solvent, dosage and administration, input error, drug incompatibility etc. Irrational selection of solvent took up the biggest proportion （42.1% ）, which mainly concentrated on platinum anticancer drugs. CONCLUTION Pharmacists can reduce the rate of irrational medication through auditing and intervening medication orders before admixture so as to ensure the medication safety of patients.
