既往血吸虫感染潜在长期效应对改善老年男性心血管病危险因素的探讨

Potential long-term effects of previous schistosome infection may help mitigate cardiovascular disease risks in elderly men

摘要 目的 分析老年男性既往血吸虫感染(PSI)的潜在长期效应与心血管病(CVD)危险因素的关系。 方法 在中国既往血吸虫感染疫区常住、年龄>65岁的体检人群中,选取PSI组211例,对照组419例作为研究对象,就腰围、血压、血糖、血脂、血尿酸等CVD危险因素进行对照研究。结果 (1)高密度脂蛋白胆固醇(HDL-C)高于对照组,三酰甘油(TG)、腰围、和体质指数(BMI)低于对照组(均P<0.001).(2)高TG血症和腹型肥胖患病率低于对照组(均P<0.001);低HDLC血症、高血糖、肥胖症患病率低于对照组(均P<0.05).(3) PSI与HDL-C呈正相关(r=0.32,P<0.001);与TG、BMI和腰围呈负相关(r=-0.36、-0.27、-0.26、均P<0.001);与尿酸、收缩压和空腹血糖呈负相关(r=-0.11、-0.08、-0.08、均P<0.05)。 结论 PSI潜在的长期效应可改善老年男性CVD危险因素。 Objective To investigate the association between potential long-term effects of previous schistosome infection (PSI) and certain risk factors for cardiovascular disease (CVD) in elderly men.Methods Of residents aged 66 years or over living in previous schistosome-endemic regions of China and receiving regular health examinations,211 with PSI were selected as the study subjects (the PSI group),while 419 without PSI served as controls (the control group).Risk factors for CVD,including waist circumference (WC),blood pressure,blood glucose concentration,blood lipid level,and serum uric acid (UA) level,were analyzed and compared between the PSI group and the control group.Results (1) The level of high-density lipoprotein cholesterol (HDL-C) was significantly higher in the PSI group than in the control group (P〈0.001),while the triglyceride (TG) level,WC and body mass index (BMI) were significantly lower in the PSI group than in the control group (P〈0.001); (2) The prevalences of hypertriglyceridemia and abdominal obesity were significantly lower in the PSI group than in the control group (P〈0.001 for both).Likewise,the prevalences of low HDL-C,hyperglycemia and obesity were significantly lower in the PSI group than in the control group (P〈0.05 for all); (3) PSI was positively correlated with the HDL-C level (r=0.32,P〈0.001) and negatively correlated with the TG level,BMI,WC,UA,DBP and FPG level (r =-0.36,-0.27,-0.26,-0.11,-0.08 and-0.08,respectively,P〈0.001 or 0.05 for all).Conclusions Potential long-term effects of PSI can impact certain risk factors for CVD and may have preventive effects on the development of CVD.
