钻孔引流联合颅内压监测治疗高血压脑出血的临床观察 被引量:11

摘要 目的探讨钻孔引流联合颅内压监测指导治疗高血压脑出血的临床疗效。方法回顾性分析32例资料完整的高血压脑出血病人,均采用钻孔引流联合颅内压监测治疗。观察手术时间、引流术后的血肿清除率、颅内压值、甘露醇使用病例数、神经外科重症监护住院天数及预后。结果脑室探头血肿穿刺引流23例,血肿穿刺联合对侧脑室探头脑室引流9例。平均手术时间28.3 min,术后24 h内CT复查平均血肿清除率65.25%,NICU病房平均住院天数6.3 d,使用甘露醇治疗7例,术后颅内压<20 mmHg 30例,≥20 mmHg 2例。术后6个月恢复良好20例,中残6例,重残2例,植物生存2例,死亡2例。结论高血压脑出血钻孔引流联合颅内压监测能指导控制引流量,避免颅内压波动和再出血风险,指导药物治疗来降低并发症,尽早发现病情变化并改善预后,两者有效结合是高血压脑出血重要的微创治疗措施之一。
