Validation of standardized uptake values for 18F-FDG in normal organs: comparison of whole-body PET/CT and PET/MRI

摘要 目的 通过PET/CT与PET/MRI在人体器官放射性摄取值(SUVmax)的配对比较,评价PET/MRI定量分析的能力.方法 选择2012年5月至2013年2月解放军总医院同日PET/CT和PET/MR检查且临床及影像学资料完整的277例患者.以右肺、肝右叶,第3腰椎椎体及竖脊肌为参考区,将感兴趣区(ROI)分别置于PET衰减校正后图像,获得SUVmax-CT和SUVmax-MRI.相同的ROI也置放于双时间点PET/CT图像的肝脏和竖脊肌.采用配对符号秩和检验以及Spearman相关性分析评价两组SUV max之间的关系.结果 57例受试者未见异常征象,220例患者中经病理证实259例恶性及21例良性病灶.除了肺部(r=0.44),SUVmax-CT和SUVmax-MRI在其他3个器官均存在高度相关(r =0.62~0.73).4个组织器官SUVmax-MRI均明显低于SUVmax-CT(P<0.01).延迟校正后,肝脏及肌肉两组SUV之间差异仍有统计学意义(P<0.01).结论 尽管SUVmax-MRI与SUVmax-CT存在差异,但是两者高度相关,提示PET/MRI均能提供可靠的定量诊断信息. Objective To validate whether PET/MR could provide a semi-quantitative measurement (SUVmax) comparable to that produced by PET/CT in normal organs.Methods 277 subjects underwent an ordinary 18F-FDG PET/CT followed by a PET/MR scan with a 25-45 min interval.Region of interest (ROIs) were drawn in 4 reference normal organs/tissues in both MRAC-PET and CTAC-PET images and the liver and erector spinae in the dual-time point PET/CT images.Results 259 malignant and 21 benign lesions,pathologically confirmed,were detected in the 220 subjects.SUWmax derived from PET/CT (SUVmax-CT) and PET/MR (SUVmax-MRI) was highly correlated over the reference organ ROIs (r =0.62-0.73),except lung (r =0.44).The SUVmax-MRI was significantly lower than the respective SUVmax-CT in all 4 organs and after delay-correction in liver and muscle.Conclusion The results indicate that PET/MR can provide reliable measurement in physiological organs.
