临床药师对肿瘤科不合理用药的干预分析

Intervention of Clinical Pharmacists on the Irrational Drug Use in Oncology Department

目的了解临床药师对肿瘤科不合理用药的干预情况。方法收集2013年7月至2014年4月临床药师对肿瘤科医嘱审核干预记录共96例次,按用法用量、溶媒品种及用量、联合用药、药物选择等进行整理并分析。结果干预较多的类别有溶媒品种不适宜(21.9%)、用法用量超说明书(18.8%)及联合用药不适宜(12.5%)等。其中,对溶媒用量及药物选择不适宜的干预建议采纳率高(100.0%),而联合用药不适宜的干预建议采纳率较低(58.3%)。结论临床药师在医嘱审核中能起到一定的作用,但仍应加强学习,对干预内容应有充分依据和理由,才能提高医师的接受率。

Objective To investigate the intervention of clinical pharmacist on irrational drug use in the oncology.Methods From July 2013 to April 2014,96 cases of medical record which were intervened by clinical pharmacists in oncology department were collected and analyzed according to the usage and dosage,menstruum type and quantity,drug combination and drug selection of the intervention.Results The major components of intervention included the inappropriate menstruum varieties（21.9%）,usage and dosage beyond the drugs instructions（18.8%） and inappropriate drug combination（12.5%）,etc.Meanwhile,doctors took advice of clinical pharmacists mostly in solvent volume and drug selection（100.0%）,but less in drug combination（58.3%）.Conclusion Clinical pharmacists could play a certain role in the audit,but they still should strengthen their professional knowledge.The intervention of clinical pharmacists should be based on sound evidence and reasonable guidelines so as to improve the doctors＇ acceptance rate.
