布地奈德对COPD急性发作期肺泡灌洗液中IL-8表达的影响

The effect of Budesonide on the expression of IL-8 in bronchoalveolar lavage in acute exacerbations of COPD

目的探讨COPD患者BALF中IL-8水平及布地奈德对其调节作用。方法选择本科2009年7月-2013年7月COPD急性发作期患者按就诊时间分为常规组和治疗组各60例。常规组予综合治疗,治疗组在此基础上进行布地奈德雾化吸入治疗,疗程均为3周,对2组治疗前、治疗1周、治疗3周BALF中IL-8水平进行检测,并与GOLD分级进行相关性分析。结果治疗前、治疗1周、治疗3周IL-8浓度常规组分别为(9.68±1.46)pg/ml、(9.45±1.63)pg/ml、(8.70±2.00)pg/ml,治疗组分别为(9.35±1.49)pg/ml、(8.75±1.57)pg/ml、(6.27±1.96)pg/ml,治疗前2组差异无统计学意义(P>0.05);治疗1周后2组差异有统计学意义(P<0.05),治疗3周后2组差异有统计学意义(P<0.01);BALF中IL-8水平与GOLD分级呈正相关(r=0.519,P<0.01)。结论 COPD患者BALF中IL-8能反映COPD气道炎症情况,布地奈德能显著降低BALF中IL-8水平。

Objective To observe the expression of IL- 8 and impaction of Budesonide in bronchoalveolar lavage in acute exacerbations of chronic obstructive pulmonary disease. Methods The patients hospitalized in our department from July 2009 to July 2013 withthe acute attack of chronic obstructive pulmonary disease were selected and divided into the conventional group and the treatment group according to visiting time,60 cases each. The conventional group were treated with comprehensive therapy and the treatment group were treated with budesonide inhalation therapy on basis of the conventional group,and the course of treatment were 3 weeks. The levels of IL- 8 in BALF were measured before treatment and 1 weeks,3 weeks of treatment,and the correlation between the levels of IL- 8 and GOLD was analysed by grades. Results The level of IL- 8 in before treatment,1 week treatment and3 weeks of treatment were（ 9. 68 ± 1. 46） pg /ml,（ 9. 45 ± 1. 63） pg /ml,（ 8. 70 ± 2. 00） pg /ml in the conventional group respectively,and（ 9. 35 ± 1. 49） pg /ml,（ 8. 75 ± 1. 57） pg /ml,（ 6. 27 ± 1. 96） pg /ml in the treatment group respectively. The level of IL- 8 in the conventional group and the treatment group,there were no significant difference between them before treatment（ P 〉0. 05）; significant differences after 1 weeks of treatment（ P〈 0 05） and more significant differences after 3 weeks of treatment（ P〈 0 01）. and the concentration of IL- 8 in BALF were positively correlated with GOLD（ r =0.519,P〈 001）. Conclusion The level of IL-8 can reflect airway inflammation of chronic obstructive pulmonary disease,which can be significantly decrease by budesonide.
