摘要
目的观察可溶性糖基化终末产物受体(sRAGE)在重型颅脑损伤应激性高血糖患者中的表达情况及其与28 d死亡率的相关性。方法采用酶联免疫技术测定48例重型颅脑损伤患者血清sRAGE浓度并记录患者临床一般资料,根据28 d预后情况将重型颅脑损伤患者分为死亡组和存活组,比较两组患者血清sRAGE水平差异,分析sRAGE水平与28 d死亡率之间的相关性。结果重型颅脑损伤患者血清sRAGE浓度高于对照组,差异具有显著性(P<0.01);死亡组与存活组比较,死亡组患者血清sRAGE水平低于存活组(P<0.01);死亡相关危险因素Logistic回归分析显示高血糖、低sRAGE水平及高APACHE-Ⅱ评分为影响预后的危险因素。结论重型颅脑损伤患者血清sRAGE表达升高,且存活组高于死亡组,低水平sRAGE可能与重型颅脑损伤患者预后不良有关。
【Objective】To study the serum level of soluble receptor for advanced glycation end products(sRAGE)in severe traumatic brain injury patients and its relationship with 28 days mortality.【Methods】Enzyme-linked immunosorbent assay(ELISA) was used to detect the serum sRAGE level in 48 severe traumatic brain injury patients and 25 normal controls, other clinical index was also tested in severe traumatic brain injury patients. According to the patients' prognosis,the patients were divided into nonsurvivors and survivors. The serum sRAGE level and other clinical index were compared between survivors and nonsurvivors. 【Results】Serum sRAGE level was significantly higher in severe traumatic brain injury patients than normal controls(P〈0.01). The levels of sRAGE in nonsurvivors were lower than those of survivors. The regression analyses showed that the sRAGE levels were associated with 28 days mortality.【Conclusions】sRAGE is significantly higher in severe traumatic brain injury patients and correlated to 28 days mortality.
