关节融合术治疗足第一跖趾关节痛风性破坏

Arthrodesis of the first metatarsophalangeal joint for osseous erosion and joint damage in uarthritis

目的分析总结13例第一跖趾关节痛风性关节破坏行关节融合术的疗效及临床经验。方法 13例第一跖趾关节痛风性关节破坏采用可调式外固定支架固定行关节融合。结果随访13例患者至完全负重行走后27周,关节完全骨性融合时间平均19周,完全负重行走时间平均21周,AOFAS-HMI评分术前平均46分,术后平均78分。结论关节融合术治疗足第一跖趾关节痛风性关节破坏,有效地稳定关节,解除疼痛,维护内侧柱长度,矫正畸形。

[Objective] This paper presents the results of 13 consecutive first MTP joint arthrodesis pro- cedures for the treatment of joint damage in uarthritis. [Methods] First MTP joint arthrodesis was achieved with adjustable external fixation. [Results] The mean follow-up period was 27 weeks after full weight bearing walk, the mean completely bony fusion time was 19 weeks, the mean full weight beating walk time was 21 weeks. AOFAS- HMI scores rose from preoperative mean of 46 to postoperative 78 points. [Conclusion] Arthrodesis of the first MTP joint is a well-accepted surgical option for advanced joint damage in uarthtitis. This procedure predictably provides stability of the joint, eliminates the painful joint motion, stabilizes the me- dial column, and achieves permanent correction of deformity.
