新双源CT在房颤患者左房-肺静脉成像中的应用

摘要 目的探讨新双源CT在房颤患者左房-肺静脉形态评价中的价值。方法回顾分析35例行双源CT扫描房颤患者,利用多平面重建(MPR)、最大密度投影(MIP)、容积再现(VR)、仿真内镜(VE)等三维重组技术进行后处理。观察左心房、肺静脉的形态结构,总结房颤患者左心房、肺静脉的形态学特点。结果①35例患者中21例为标准4支(60%),14例有变异(40%)。双上肺静脉较双下肺静脉粗(P〈0.05),右上、下肺静脉分别较左上、下肺静脉粗(P〈0.05);两侧上下肺静脉指数差异无统计学意义(P〉0.05),左肺静脉指数与右侧比较差异有统计学意义(P〈0.05)。②35例患者中左房顶部17例为I型(48.6%),12例Ⅱ型(34.3%),6例Ⅲ型(17.1%)。③35例患者中左心耳-肺静脉界嵴A工型5例,AⅡ型13例,B工型8例,BⅡ型9例。界嵴长度16.5-42.7mm(28.1±5.4mm),宽度0.8-11.3mm(5.2±1.1mm)。④冠状窦汇入右房方向0°-135°(65.4±28.90)。结论新双源CT能够直观、清晰显示左心房、肺静脉内、外部形态,明确左心房顶部形态、界嵴形态和走行以及肺静脉开口。这些影像信息能提高射频消融成功率,减少并发症的发生。
