胫骨远端锁定加压钢板与解剖型钢板内固定治疗Pilon骨折的对比研究

Comparative study of distal tibial locking compression plate internal fixation and anatomical plate internal fixation in treating Pilon fractures

目的比较胫骨远端锁定加压钢板与解剖型钢板内固定治疗Pilon骨折的临床疗效。方法随机挑选深圳市松岗人民医院骨科2011年12月至2013年12月收治的Pilon骨折患者72例,将患者分为两组,其中观察组36例采用胫骨远端锁定加压钢板进行治疗,对照组36例采用解剖型钢板内固定治疗。结果观察组手术时间和愈合时间分别为(43.4±12.4)min和(15.8±1.9)周,对照组为(56.3±18.4)min和(17.9±2.9)周,两组比较差异有统计学意义(P<0.05);观察组优良率为91.7%(33/36),对照组优良率为77.8%(28/36),两组比较差异有统计学意义(P<0.05)。结论胫骨远端锁定加压钢板治疗Pilon骨折,手术时间和愈合时间均较解剖型钢板短,复位较为理想,固定牢靠,值得推广应用。

Objective To compare the clinical effect of distal tibial locking compression plate internal fixation and anatomical plate internal fixation in Pilon fractures .Methods A total of 72 patients with Pilon fracture from De-cember 2011 to December 2013 were randomly divided into two groups ,36 patients in observation group were treated with distal tibial locking compression plate ,and the other 36 patients in control group were treated with anatomic plate fixation .Results The operation time and healing time were （43 .4 ＆#177; 12 .4） min and （15 .8 ＆#177; 1 .9） weeks in the observation group ,and times in the control group were （56 .3 ＆#177; 18 .4） min and （17 .9 ＆#177; 2 .9） weeks ,the differences between the two groups were significant （P〈0 .05） .The excellent rate in observation group was 91 .7% （33/36） , and that in the control goup was 77 .8% （28/36） ,the difference was statistically significant （P〈0 .05） .Conclusion The operation time and healing time of patients with Pilon fractures treated by distal tibial locking compression plate internal fixation is shorter than those treated by anatomical plate internal fixation ,with more desirable reset and sta-ble fixation .
