山楂消脂胶囊对代谢综合征并颈动脉硬化患者颈动脉IMT和斑块积分的影响

Effects of Shanzha Xiaozhi Capslue on Intima-media Thickness and Plaques Score of Carotid Artery in Patients with Metabolic Syndrome and Carotid Atherosclerosis

目的:观察山楂消脂胶囊对代谢综合征(MS)并颈动脉硬化(CAS)患者颈动脉内-中膜厚度(IMT)和斑块积分的影响。方法:将符合诊断标准的82例MS并CAS患者随机分为两组。对照组40例予改善生活方式并控制好相关危险因素,治疗组42例在对照组基础上加用山楂消脂胶囊,疗程为6个月。观察心脑血管事件的发生率,同时比较两组治疗前后颈动脉的IMT和斑块积分,并测定治疗前后Hs-CRP、Hcy、HOMA-IR各指标的变化。结果:治疗组急性心脑血管事件发生率较对照组有降低的趋势,但差异无统计学意义(P>0.05)。治疗组的颈动脉IMT和斑块积分低于对照组,差异有统计学意义(P<0.05),同时治疗组可明显降低Hs-CRP、Hcy、HOMA-IR,与对照组比较,差异有统计学意义(P<0.01)。结论:山楂消脂胶囊可能通过降低代谢综合征患者颈动脉斑块中的脂质含量,抗炎症反应,抗胰岛素抵抗,减轻颈动脉内中膜的厚度和斑块积分,从而稳定斑块,减少急性心脑血管事件的发生。

To investigate the effects of Shanzha Xiaozhi Capslue on intima-media thickness and plaques score of carotid artery in Patient with MS and CAS. Methods： The 82 patients with MS and CAS randomly divided into treatment group（42 cases） and control group（40 cases）. Patients in the control group just improved their way of life and controlled the relevant risk factors, while patients in the treatment group received Shanzha Xiaozhi Capslue on the basis of the control group, for 6 months. Then the incidence rate of major adverse cardiovascular and cerebrovascular events of both groups was observed. The intima-media thickness and plaques score of carotid artery were compared, and the changes of Hs-CRP, Hcy and insulin resistance of both group were measured before and after the treatment. Results： Compared with the control group, the incidence rate of major adverse cardiovascular and cerebrovascular events in treatment group showed a tendency of decrease（P〉0.05）. The intima-media thickness and plaques score of carotid artery of the treatment group was lower than that of the control group（P〈0.05）.Compared with the control group, the treatment group significantly reduced Hs-CRP,Hcy,HOMA-IR（P〈0.01）. Conclusion： Shanzha Xiaozhi Capslue can reduce the lipid of the atherosclerotic plaque, relieve the inflammatory reaction, resist insulin resistance, decrease the intima-media thickness and plaques score of carotid artery, and thus stabilize atherosclerotic plaque, reduce the occurrence of acute cardiovascular events.
