目的对我院静脉用药调配中心不合理医嘱进行深入的分析,从中探寻合理有效地用药干预措施。方法对我院2011年3月—2013年5月用药医嘱记录资料进行回顾分析,这三年共有医嘱记录800条。从中挑选出不合理的用药医嘱500条,对这500条不合理医嘱的情况进行具体的分析对医师针对不合理医嘱所进行的措施进行探究。结果通过对不合理的医嘱采取必要的措施,使得不合理医嘱被更正或者终止的有480条。干预有效率高达96.0%,采取用药干预后大大的提高了医嘱的准确度。结论对临床不合理医嘱进行分析从中发现不合理医嘱发生的原因,并以此提出有效地用药干预措施。可以及时有效地更正或者终止不合理医嘱,能够更大程度上提高合理用药的水平。

Objective To intravenous drug control unreasonable medical center in our hospital were analyzed, to explore a reasonable effective drug interventionfrom. Methods In our hospital from 2011 March to 2013 May prescription recordswere retrospectively analyzed, this three years a total of 800 medical records.Select the irrational medication orders 500, a detailed analysis of the500 unreasonable orders of physicians for the irrational medical orders of measuresto explore. Results By taking necessary measures to unreasonableprescriptions, the irrational medical orders were corrected or terminated with 480.Intervention is effective rate reaches as high as 96%, to take the medicationintervention greatly improves the accuracy of the doctor＇s advice. Conclusion Analysis found that cause unreasonable orders from occurring on clinical unreasonable prescription, and to put forward effective intervention measures.Can effectively correct or terminate the irrational medical orders, to a greaterextent improve the level of rational drug use.
