抑郁症病人平均住院日的影响因素分析

Influence Factors Analysis on the Average Hospitalization Days of Depression Patients

目的了解抑郁症病例住院日的水平,探讨其影响因素,在一定程度上可缓解"看病难、看病贵"的问题。方法利用我院2008年-2012年抑郁病病人的病案首页信息,采用中位数和四分位数描述平均住院日,利用回归分析模型分析影响抑郁症患者住院日的因素,探讨患者年龄、性别、付费方式、入院情况、职业,婚姻状况、有无精神病症状、患有其他疾病等对住院日的影响。结果 479例病例中住院日中位数为24.5天,2008年-2012年住院日中位数有逐渐缩短趋势,通过Mann-Whitney U检验计算和Kruskal-Wallis H检验分析显示,是否初发病、疗效、婚姻和付款方式等项P<0.05,对住院病人平均住院日的影响有统计学意义。结论住院患者的支付方式和病情严重程对住院费用和住院日具有明显影响,医院必须具有以病人为中心的服务思想和行为,才能避免单纯缩短平均住院天数的偏向,实现医院的可待续发展。

Objectives To understand the average hospitalization days level of depression patients, investigate its influence factors, and alleviate the problems of difficulty and high cost in getting medical service to some extent. Methods To collect the medical record home page information of depression patients in our hospital from 2008 to 2012, describe the level of average hospitalization days using the median and quartiles, analyze the influence factors of hospitalization days with the regression analysis model, and explore the influence of patients age, gender, payment method, the hospital situation, occupation, marital status, presence of psychotic symptoms, patients with other diseases on the hospitalization days. Results The median of the hospitalization days of 479 cases of patients was 24. 5, and it had a gradually shorten trend during 2008 to 2012. The analysis results of the Mann-Whitney U test calculation and Kruskal Wallis H test showed that the factors included whether in onset, curative effect, the presence of psychotic symptoms, marriage and the terms of payment had statistically significance on the average hospitalization days （P 〈 0. 05）. Conclusions The mode of payment and the severity of diseases had obvious effects on hospitalization expenses and stay days. The hospital must have a patient-centered service thought and behavior so as to avoid the bias of simply shortening the average hospitalization days and realize the sustainable development of the hospital.
