院前急救中低血糖昏迷患者90例临床分析

Clinical analysis of 90 cases of hypoglycemia coma patients in pre-hospital emergency treatment

目的探讨院前急救中低血糖昏迷患者临床疗效影响因素。方法回顾性分析从2013年2月至2014年2月期间来我院抢救的90例低血糖昏迷患者,在治疗前测量患者外周血血糖,并予静脉滴注葡萄糖升血糖,确保患者血糖在正常范围内,院前治疗至医院时醒转患者为A组,至医院时昏迷程度较院前急救前有好转为B组,至医院时仍无变化患者为C组,统计三组患者资料并分析影响因素。结果 A组34例,B组18例,C组38例,有效率为57.78%。A组和B组患者的平均年龄要显著低于C组(P<0.05);A组以及B组患者昏迷时间也显著短于C组(P<0.05)。A组患者血糖浓度显著高于B组、C组(P<0.05)。结论低血糖昏迷患者的年龄、血糖浓度、昏迷时间等因素均能影响院前急救的疗效,故应根据患者的具体病情、一般资料进行评估,以确保急救效果。

ObJective：To investigate the prehospital factors which influence the clinical effect on hypoglycemic coma patients. Methods：Ninety cases of hypoglycemia during the period from 2013 February to 2014 February in our hospital were considered retrospectively. Blood glucose of patients was measured before treatment,and treated with intravenous infu-sion of glucose glucagon to ensure the blood sugar in normal range. They were divided into Group A of the patients with an-alepsia upon arrival at the hospital after pre-hospital treatment,Group B of patients with coma improvement upon arrival at the hospital after pre-hospital treatment and Group C of patients with no change upon arrival at the hospital after pre-hospital treatment. Statistical data of three groups and influencing factors were analyzed. Results：Thirty-four cases were in the A group,18 cases in the B group and 38 cases in C group,and the efficiency was 57. 78% . The average age and coma dura-tion of the patients in the A group and the B group was significantly shorter that in the C group（P 〈 0. 05）. Blood glucose concentration of the group was significantly higher than that in the B group and the C group（P 〈 0. 05）. Conclusion：Age, blood glucose concentration,coma time and other factors can affect the pre hospital first aid of hypoglycemic coma patients, which should be assessed according to the patientˊs specific condition and general information to ensure the aid effect.
