血小板耗竭在大鼠呼吸机诱导肺损伤中的保护作用

摘要 目的:探讨血小板耗竭是否对大鼠呼吸机诱导的肺损伤存在保护作用。方法:实验大鼠随机分成对照组(C组)、低压机械通气组(L组)、高压机械通气组(H组)和高压机械通气合并血小板耗竭组(P组)。低压和高压机械通气参数分别为吸气压力(PIP)1.5kPa、呼气末正压(PEEP)0.2kPa和PIP 3.0kPa、PEEP 0kPa。腹腔注射兔抗鼠血小板血清用于耗竭血小板。所有实验大鼠均检测动脉血气分析,肺组织干/湿比(W/D)、肺组织丙二醛(MDA)、髓过氧化物酶(MPO)和组织形态学。结果:H组大鼠相比C组氧合能力更差[PaO2(6.10±1.74)kPa与(15.12±1.23)kPa(P<0.05)],P组相对H组氧合能力有所改善[PaO2(11.08±1.44)kPa与(6.10±1.74)kPa(P<0.05)]。相比C组,H组大鼠肺组织损伤明显,表现为W/D更高[4.44±0.37与2.12±0.55(P<0.05)],MDA[(3.21±0.4)nmol/g与(1.22±0.26)nmol/g(P<0.05)]和[MPO(5.45±1.14)U/g与(3.34±0.59)U/g(P<0.05)]浓度更高,而P组肺损伤显著改善[W/D 3.23±0.51与4.44±0.37(P<0.05)];MDA(2.57±0.34)nmol/L与(3.21±0.4)nmol/l(P<0.05);MPO(4.63±0.85)U/g与(5.45±1.14)U/g(P<0.05)。肺组织形态学检查P组相对H组,肺水肿程度显著下降(P<0.05)。结论:血小板耗竭能够显著改善大鼠呼吸机诱导的肺损伤严重程度。
