多项血清指标联合分析在冠心病诊断中的应用

Combined analysis of serum multi-indicator in the diagnosis of coronary heart disease

目的探讨血清多指标联合分析在冠心病(CHD)诊断中的应用。方法分别采用免疫比浊法、酶法、选择性抑制法,对153例CHD患者(CHO组)及350例健康人群(对照组)的血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白(Apo)A1、ApoB、脂蛋白a(Lpa)、尿酸(UA)、同型半胱氨酸(Hcy)、D-二聚体(D-D)和超敏C反应蛋白(hs-CRP)水平进行分析测定。结果稳定型心绞痛(SAP)组除TC、HDL-C、ApoA1、ApoB、UA外,其余指标均明显高于对照组(P<0.05);不稳定型心绞痛(UAP)组除HDL-C、ApoA1、UA外,其余指标均明显高于对照组(P<0.05);急性心肌梗死(AMI)组除HDL-C外,其余指标均明显高于对照组(P<0.05)。结论多项血清学指标联合分析,对CHD患者进行风险评估、临床诊断及预后判断具有重要意义。

Objective To discuss the significance of combined analysis of serum multi-indicator in the diagnosis of coronary heart disease（CHD） .Methods Total cholesterol（TC） ,triacylglycerd（TG） ,high density lipoprotein cholesterol（HDL-C） ,low density lipoprotein cholesterol（LDL-C） ,apolipoprotein（Apo）A1 ,ApoB ,lipoprotein a（Lpa） ,uric acid（UA） ,homocysteine（Hcy） ,D-dimmer （D-D） and hypersensitive C-reactive protein（hs-CRP） levels of 153 patients with CHD and 350 healthy control subjects（control group） had been determined by immunonephelometry ,enzyme assay and selective inhibition assay .Results The indicators of stable angina pectoris（SAP） group were significantly higher than those of control group（P〈0 .05） ,except TC ,HDL-C ,ApoA1 ,ApoB and UA .The indicators of unstable angina pectoris（UAP） group were significantly higher than those of control group（P〈0 .05） ,ex-cept HDL-C ,ApoA1 and UA .The indicators of acute myocardial infarction（AMI） group were significantly higher than those of control group（P〈0 .05） ,except HDL-C .Conclusion Combined analysis multiple serum indicators has great significance on risk as-sessment ,clinical diagnosis and prognosis judge in patients with CHD .
