自体皮质骨板移植及人工骨粒植骨加可调式外固定支架治疗儿童严重开放粉碎性胫骨骨缺损分析 被引量:1

摘要 目的:探讨儿童严重开放粉碎性胫骨骨折尤其合并骨缺损、肢体短缩的治疗方法。方法:对14例严重开放粉碎性胫骨骨折并骨缺损的患儿在术前和术后应用有效抗生素的基础上,1期彻底清创缝合,待软组织条件成熟后2期依骨缺损的长度进行自体皮质骨板移植及人工骨粒植骨,加上可调式外固定支架固定;术后严格无菌换药,并使用敏感抗生素,平均用药12也1天。结果:全部病例均于术后8—14周达到治愈出院,平均6.8周胫骨达到骨性愈合,患肢长度短缩约0~1.8cm,14例患者3个月后均可逐渐下地负重,进行功能锻炼后无明显跛行表现。结论:可调式外固定器结合自体皮质骨板移植及人工骨粒植骨是治疗儿童严重开放粉碎性胫骨骨缺损的简单、积极而有效的方法。
