腹腔镜TME与开腹TME治疗直肠癌效果分析

Analysis of the Effect of Laparoscopic TME and Open TME on Rectal Cancer

目的探讨针对直肠癌患者行开腹全直肠系膜切除术(TME)与腹腔镜TME的有效性与安全性。方法选取该院2012年1月—2014年1月收治的直肠癌患者94例,随机将样本分为对照组与实验组各47例,对照组患者给予开腹TME治疗,实验组患者给予腹腔镜TME治疗,观察两组样本的治疗效果以及并发症情况,并开展比较与分析。结果两组患者的淋巴结清扫数目比较差异无统计学意义(P>0.05),实验组患者的手术时间为(174.8±59.3)min,出血量为(85.9±29.4)mL、切口愈合时间为(8.3±0.9)d、肠道功能恢复时间为(46.8±15.5)h、住院时间为(9.4±4.3)d、无并发症发生;对照组患者手术时间为(119.4±39.8)min,出血量为(123.4±29.8)mL、切口愈合时间为(12.8±1.1)d、肠道功能恢复时间为(91.7±37.9)h、住院时间为(14.3±5.7)d、并发症发生率为10.6%,两组间比较实验组均显著好于对照组(P<0.05)。结论针对直肠癌患者开展腹腔镜TME手术与开腹TME手术所达到的清除效果相当,但腹腔镜手术对患者所产生的创伤较小。

Objective To explore the effectiveness and safety of open total mesorectal excision （TME） and laparoscopic TME in the treatment of rectal cancer patients. Methods 94 cases with rectal cancer admitted in our hospital from January 2012 to January 2014 were selected and randomly divided into the control group and the experimental group with 47 cases in each. The control group were treated with open TME treatment, and the experimental group were treated with laparoscopic TME treatment. The treatment effects and complications of the two groups of samples were observed, compared and analyzed. Results There was no statistically significant difference in the number of lymph node dissection between both groups （P〉0.05）. The operative time in the experimental group was （174.8±59.3）min, blood loss was （85.9±29.4）mL, wound healing time was（8.3±0.9）d, intestinal function recovery time was （46.8±15.5）h, hospital stay was （9.4±4.3）d and no complications occurred. The operative time in the control group was （ 119.4 ±39.8 ） min, blood loss was （ 123.4 ±29.8 ） mL, wound healing time was （ 12.8±1.1 ） d, intestinal function recovery time was （91.7±7.9）h, hospital stay was （14.3±5.7）d and the complication rate was 10.6%, the experimental group was much better than the control group by comparison with statistical significance （P〈0.05）. Conclusion For patients with rectal cancer, the scavenging effect of laparoscopic TME is similar to that of open TME, but the trauma caused by laparoscopic TME is smaller.
