中药药浴联合丹参注射液治疗泛发性斑块型银屑病53例临床观察

Clinical Observation on 53 Generalized Plaque Psoriasis Patients Treated by Chinese Medicine Bath Therapy Combined with Danshen Injection

目的观察丹参祛瘀洗剂药浴联合注射用丹参静脉滴注治疗泛发性斑块型银屑病的疗效。方法将89例泛发性斑块型银屑病患者随机分成观察组53例,对照组36例。观察组给予丹参祛瘀洗剂药浴(每次20min,每日1次)联合注射用丹参静脉滴注(400mg/d),对照组每日午餐后口服阿维A胶囊,初始剂量20mg/d,最大剂量不超过40mg/d。两组皮疹处均采取卡泊三醇膏封包、莫米松乳膏涂擦。两组均于治疗后第3天、第1周、第2周评价临床疗效,比较治疗后第3天、第1周、第2周银屑病面积和严重指数(PASI)评分。结果观察组治疗3天、1周、2周PASI评分均较治疗前明显下降(P<0.05);对照组治疗1周、2周PASI评分均较治疗前明显下降(P<0.05);两组治疗后3天、1周、2周PASI评分比较差异无统计学意义(P>0.05)。观察组治疗3天总有效率为13.21%,对照组为8.33%,两组比较差异有统计学意义(P<0.05),治疗1周及2周,两组总有效率比较差异无统计学意义(P>0.05)。结论丹参祛瘀洗剂药浴联合注射用丹参静脉滴注治疗泛发性斑块型银屑病临床疗效和阿维A胶囊相当,且起效快。

Objective To observe the therapeutic effect of Chinese medicine bath therapy combined with Dansh- en Injection intravenous drip in treating generalized plaque psoriasis. Methods Totally 89 cases of generalized plaque psoriasis were randomized into the observation group of 53 cases and control group of 36 cases. Patients in the observation group were treated by Chinese medicine bath therapy （Danshen stasis-removing lotion） together with Danshen intravenous drip. The patients in the control group were orally taken acitretin capsules. In both groups, rash area was bandaged with calcipotriol cream and applied with mometasone furoate cream. The clinical efficacy of both groups was observed after 2-week treatment. After treatment, Psoriasis area and severity index （PASI） scores of the 3rd day, the 1 st week and the 2nd week were compared. Results PASI scores of the observation group after three-day, oneweek and two-week treatment were significantly decreased as compared with those before treatment （ P 〈 0. 05 ）. PASI scores of the control group after one-week and two-week treatment were significantly decreased as compared with those before treatment （P 〈 0. 05 ）. There was no significant difference between groups in PASI scores after three-day, one- week and two-week treatment （ P 〉 0. 05 ）. The total effective rate of the observation group and control group after three-day treatment was 13.21% and 8.33% respectively, with a statistical significance between groups （P 〈 0. 05 ）. There was no significant difference between groups in the total effective rate after one-week and two-week treatment （P 〉 0. 05 ）. Conclusion Compared with acitretin capsules, Chinese medicine bath therapy combined with Danshen intravenous drip has similar clinical effect in treating generalized plaque psoriasis. In addition, this method has few side-effects but quick action.
