慢性阻塞性肺疾病患者血清甲状腺激素水平变化的临床意义 被引量:11

Clinical significance of thyroid related hormones in patients with chronic obstructive pulmonary disease

摘要 目的 观察慢性阻塞性肺癌病(COPD)患者血清甲状腺激素水平的变化,探讨其临床意义.方法 将兰州军区兰州总医院2012年1月至2013年1月收治的96例COPD患者纳入COPD组,并按病情分为3个亚组,COPD缓解期组(30例)、COPD急性发作无呼吸衰竭组(31例)和COPD急性发作并呼吸衰竭组(35例);选择同期体检健康者90例作为正常对照组.检测各组甲状腺激素水平.结果 对照组血清总三碘甲状腺原氨酸(TT3)、总四碘甲状腺原氨酸(TT4)、促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4)分别为(1.5±0.5) nmol/L、(96±36) nmol/L、(3.3±1.2) mU/L、(4.4±1.2)pmol/L、(16.6±9.6)pmol/L;COPD组TT3、TT4、TSH、FT3、FT4分别为(0.7±0.3)nmol/L、(66±33)nmol/L、(2.5±1.2) mU/L、(2.1±0.5)pmol/L、(12.1±6.6) pmol/L; COPD组TT4、FT4、TSH、TT3、FT3明显低于正常对照组(P<0.01或P<0.05).COPD缓解期组TT3、T4T、TSH、FT3、FT4分别为(1.13±0.59)nmol/L、(86±28)nmol/L、(3.0 ±0.8) mU/L、(3.2±1.1)pmol/L、(14.3±6.6) pmol/L; COPD急性发作无呼吸衰竭组分别为(0.77±0.45) nmol/L、(66±24) nmol/L、(2.6±0.7) mU/L、(1.9±0.8) pmol/L、(11.3±6.6) pmol/L;COPD急性发作并呼吸衰竭组分别为(0.45±0.24) nmol/L、(44±25)nmol/L、(1.2±0.6)mU/L、(1.5±0.7)pmol/L、(7.9±3.4)pmol/L;COPD急性发作无呼吸衰竭组TT3、FT3明显低于COPD缓解期组(P<0.05),TT4、FT4、TSH差异无统计学意义(P>0.05),COPD急性发作并呼吸衰竭组TT3、FT3、TT4、FT4、TSH均明显低于COPD缓解组,差异均有统计学意义(均P<0.01).结论 甲状腺激素水平可能与COPD患者的病情有密切关系,测定甲状腺激素水平可用于评估COPD患者病情的严重程度,病情越重,甲状腺激素水平下降幅度越大. Objective To explore clinical significance of serum levels of thyroid hormones in patients with chronic obstructive pulmonary disease(COPD).Methods Ninety-six cases of patients with COPD were divided into three groups according to the severity of disease.Serum thyroid hormone include levels of Serum triiodothyronin(TT3),tetraiodothyronin(TT4),free triiodothyronin (FT3),free tetraiodothyronin (FT4) and thyroid stimulating hormone(TSH) were measured by chemiluminescence immunoassay in 96 COPD patients and 90 healthy subjects as controls.Results TT3,TT4,TSH,FT3,FT4 in control group were (1.5 ± 0.5) nmol/L,(96 ± 36) nmol/L,(3.3 ± 1.2) mU/L,(4.4 ± 1.2) pmol/L,(16.6 ± 9.6) pmol/L,respectively ; those in COPD group were (0.7 ±0.3) nmol/L,(66 ± 33) nmol/L,(2.5 ± 1.2) mU/L,(2.1 ± 0.5) pmol/L,(12.1 ± 6.6) pmol/L; the levels of TT4,FT4,TSH of COPD were decreased compared with control group (P 〈 0.05) ; the levels of TT3,FT3 of COPD were decreased significantly compared with control group(P 〈 0.01).Compared with the group of remission period,the levels of TT3,FT3 of were decreased in the patients of acute period(P 〈 0.05) ; the levels of TT3,FT3 of were decreased significantly in the cases of COPD with respiratory failure than the other cases of COPD (P 〈 0.01).Conclusion Thyroid hormone levels might be involved in state of COPD; thyroid hormone levels can be used to assess the severity of COPD.
