摘要
目的 检测早产儿生后血清胰岛素样生长因子-1(IGF-1)的浓度变化,探讨其与早产儿视网膜病(ROP)发生的关系.方法 以我院收治的胎龄<34周或出生体重<2000g的早产儿为研究对象,按有无ROP发生分组,有ROP的30例设为ROP组,无ROP的选30例设为早产对照组.所有入选研究对象分别在生后1周及纠正胎龄32周、34周、36周、40周时收集外周静脉血,检测血清IGF-1水平.同时选取30例健康足月新生儿作为足月对照组.结果 两组早产儿在纠正胎龄40周前血清IGF-1水平均随周龄的增长呈上升趋势.ROP组在生后1周及纠正胎龄32、34、36周时血清IGF-1水平均明显低于早产对照组(P<0.05).ROP组及早产对照组在生后第1周时IGF-1浓度低于足月对照组(P<0.05).在纠正胎龄40周时三组IGF-1水平差异无统计学意义(P>0.05).结论 早产儿生后早期低IGF-1水平可能导致早产儿ROP的发生,于生后早期提高早产儿血中IGF-1浓度或许能有效防治ROP的发生发展.
Objective To determine the changes of serum levels of insulin-like growth factor-1 (IGF-1) in the premature infant,and to explore the correlation between serum IGF-1 and retinopathy of prematurity (ROP).Methods Premature infants with gestational age <34 weeks or birth weight <2000 g were enrolled in the study.They were divided into two groups,ROP group (with ROP) 30 cases,premature control group (without ROP) 30 cases.In the time of first week after delivery,corrected gestational age at 32,34,36 and 40 week,we collected peripheral venous blood and measured the levels of serum IGF-1.At the same time 30 cases of full term newborn were chosen as mature control group.Results Before corrected gestational age of 40 week,serum levels of IGF-lwere rising in ROP group and premature control group.IGF-1 levels of ROP group were significantly lower than those of the premature control group in the first week and corrected gestational age at 32,34,36 week (P < 0.05).IGF-1 levels in ROP group and premature control group were both lower than those of the mature control group in the first week (P < 0.05).At corrected gestational age of 40 week,there was no significant difference among three groups (P > 0.05).Conclusion Low levels of IGF-1 in premature infant may lead to ROP generation in the early period after birth.Improving the serum level of IGF-1 may effectively control the occurrence of ROP during the critical period.
