口外颈牵引配合2×4矫治技术治疗替牙期安氏Ⅱ~Ⅰ错的临床研究

Clinical study of 2 × 4 appliance and cervical headgear correction of Angle Ⅱ~Ⅰmalocclusion in mixed dentition

目的研究口外弓配合2×4矫治技术在替牙期AngleⅡⅠ错矫治中的作用。方法选择替牙期AngleⅡⅠ错12例,其中男7例,女5例,平均年龄9.6岁。上颌采用2×4矫治技术,配合口外弓颈牵引治疗。治疗前后拍摄头颅侧位片,对X线头影测量结果用SPSS 11.5统计软件进行分析。结果治疗结束时SNA及ANB角明显减小,UⅠ/SN显著变小,前牙覆盖平均减小;磨牙关系为Ⅰ类关系。鼻唇角增大,软组织侧貌明显改善。结论口外弓颈牵引配合2×4矫治技术非常适用于替牙期AngleⅡⅠ错伴上前牙轻中度拥挤的患者,它们能有效地抑制上颌的发育,减小恒牙期的治疗难度,可在临床上广泛使用。

Objective To study the soft and hard tissue changes of Angle ⅡⅠ cases in mixed dentition after the 2 × 4 appliance and cervical headgear orthodontic treatment. Methods A total of 12 Angle ⅡⅠ malocclusion patients in mixed dentition （7 males and 5 females） were selected as the subjects （9.6 years old on average）. All the patients were treated with 2 × 4 appliance and cervical headgear. The eephalometric changes were performed before and after the treatment. Statistical analysis was performed by using SPSS 11.5 software. Results After the treatment, SNA and the labial inclination of upper incisors were reduced; the nasolabial angle and ULP were increased. Conclusions 2 × 4 appliance and cervical headgear is a good choice for the patients with Angle ⅡⅠ malocclusion in mix dentition and with upper anterior crowding. It can restrain the maxilla growth, and be applied in clinic widely.
