腰椎髂骨固定螺钉治疗腰椎骨折患者的术后护理

Application of comprehensive nursing in patients with lumbar fractures after the treatment of lumbar iliac fixed screws

目的：探讨综合护理在行腰椎髂骨固定螺钉治疗的腰椎骨折患者术后护理中的应用。方法选择行腰椎髂骨固定螺钉治疗的腰椎骨折患者156例，按照随机数字表法分为观察组和对照组各78例。对照组患者术后接受常规护理，观察组患者接受综合护理干预，比较两组患者疗效和术后并发症发生率。结果观察组患者愈合时间为（14．1±2．3）周，明显短于对照组的（16．2±2．0）周，差异有统计学意义（t＝-6．085，P＜0．05）；观察组患者焦虑自评量表（SAS）评分、关节功能（Neer）评分分别为（52．9±3.5），（85．4±3．2）分，明显高于对照组的（57．2±3．9），（81．5±2．8）分，差异有统计学意义（t值分别为-7．247，8．101；P＜0．05）；观察组患者总治疗有效率为84．6％，明显高于对照组的70．5％，差异有统计学意义（χ2＝4．457，P＜0．05）；观察组患者术后感染发生率为1．3％，显著低于对照组的9．0％，差异有统计学意义（χ2＝4．743，P＜0．05）。结论对行腰椎髂骨固定螺钉治疗的腰椎骨折患者实施术后综合护理，可以改善手术疗效，降低术后并发症的发生率。

Objective To investigate the application of comprehensive nursing in patients with lumber fractures after the treatment of lumber iliac fixed screws .Methods A total of 156 patients with lumber fractures after the treatment of lumber iliac fixed screws were selected as the study object , and were randomly divided into the observation group and the control group , with 78 cases each .The control group was given the routine nursing, while the observation was given the comprehensive nursing besides the routine nursing care .The efficacy and postoperative complications of the two groups were observed and compared .Results After the nursing intervention, the fracture healing time of the observation was （ 14.1 ±2.3 ） week, which was significantly shorter than （16.2 ±2.0） week of the control group （t=-6.085,P〈0.05）.The SAS and Neer Scores were （52.9 ±3.5） and （85.4 ±3.2） of the observation group, which were significantly higher than （57.2 ±3.9） and （81.5 ±2.8） of the control group （t=-7.247,8.101, respectively;P〈0.05）.The treatment success rate of the observation group was 84.6%, which was significantly higher than 70.5% of the control group （χ2 =4.457,P 〈0.05）.The infection rate of the observation group was 1.3%, which was significantly lower than 9.0%of the control group （χ2 =4.743,P〈0.05）.Conclusions The comprehensive nursing can improve the efficacy of lumber iliac fixed screws , and reduce the occurrence of postoperative complications .It is deserved to be promoted widely in the clinical nursing work .
