超声诊断急性阑尾炎价值的Meta分析

THE CLINICAL VALUE OF ULTRASOUND IN THE DIAGNOSIS OF ACUTE APPENDICITIS:A META-ANALYSIS

目的探讨超声对于诊断急性阑尾炎（acute appendicitis,AAD）的临床应用价值。方法网络检索相关数据库PubMed（1966年—2012年12月）、EMBASE（1974年—2012年12月）、Cochrane（2012年第4期）、CBM（1978—2012年）、中国期刊全文数据库CNKI（1994年—2012年12月）等。收集多普勒超声检查诊断AAD的临床研究,按QUADAS条目评价纳入研究质量,应用统计软件STATA11.0进行数据分析。结果最终纳入7个研究,均为前瞻性队列研究,超声检查诊断AAD的敏感度、特异度为：82%（95%CI79%～85%）和84%（95%CI79%～88%）;诊断的阳性似然比（＋LR）、阴性似然比（-LR）为：4.71（95%CI2.38～9.34）,0.28（95%CI0.21-0.37）,诊断实验的SROC曲线下面积分别为：0.85（95%CI0.82～0.88）。结论多普勒超声诊断AAD的敏感性和特异性尚可,临床可采用多普勒超声诊断结果为参考结合患者症状对AAD作出准确诊断。

Objective To evaluate the diagnostic value of ultrasonography for acute appendicitis.Methods By searching Medline EMBSE,CENTRAL（the Cochrane central register of controlledtrials）,CBM,CNKI and WANFANG et al,we collected all diagnostic tests about ultrasonography for acute appendicitis.QUADAS items were used to evaluate the quality of the included studies.The sensitivity,specificity and SROC curves were calculated to assess the diagnostic value of individual diagnostic tests.The data was analyzed by using statistic software Meta-DiSc1.4.Results Total 7 trials of prospective cohort study were analyzed in this meta-analysis.The summary values of sensitivity,specificity,positive likelihood ratio,negative likelihood ratio and SROC curve of ultrasonography were 82% （95%CI79%～85%）,84% （95%CI 79%～88%）,4.71（95%CI2.38～9.34）,0.28 （95%CI0.21～0.37） and 0.85 （95%CI 0.82～0.88）.Condusion Ultrasonography can be regarded as an effective and feasible method for detection of acute appendicitis.
