摘要
目的:研究贵阳地区汉族人群中人类血小板抗原(HPA)1~5基因多态性与2型糖尿病合并颈动脉粥样硬化的相关性。方法2型糖尿病患者99例(糖尿病组),按有无颈动脉粥样硬化分为CA (+)组( n=56)和CA (-)组( n=43)两个亚组,同期健康体检者100例作为正常组,采用酚-氯仿法提取基因组DNA,序列特异性引物-多聚酶链反应(SSP-PCR)技术扩增目的基因,进行HPA基因分型。结果正常组、CA(+)组、CA(-)组三组间的HPA-1、2、3、5等位基因频率及基因型比较,差异均有统计学意义( P均<0.05);正常组与糖尿病组的HPA-1、2、3等位基因频率及基因型比较,差异均有统计学意义(P均<0.05);CA(+)组与CA(-)组的HPA-5等位基因频率及基因型比较差异有统计学意义(P<0.05)。经Logistic回归分析,年龄、糖尿病病程、高血压、吸烟、超重、血脂异常、HPA-5基因多态性为2型糖尿病患者颈动脉粥样硬化发生的危险因素( P均<0.05)。结论 HPA-1、2、3基因多态性与贵阳地区2型糖尿病的发生可能存在相关性,HPA-5基因多态性可能是2型糖尿病合并颈动脉粥样硬化发生的危险因素。
Objective To study the association between genetic polymorphism of human platelet antigens HPA -1-5 and type 2 diabetes combined with carotid atherosclerosis ( CA) of Han people in Guiyang .Methods 99 patients with type 2 diabetes (diabetes group) and 100 healthy controls (normal group) were enrolled in this case-control study.The di-abetes group was devided into two groups , which was CA( +) (combined with CA) and CA( -) (non-CA).Phenol-chloroform method was used to extract the genomic DNA and sequence-specific primers-polymerase chain reaction ( SSP-PCR) technique was applied for amplification of the target gene and genotyping .Results The allele frequency and geno-type of HPA-1, 2, 3, 5 showed significant differences among the normal group , CA(+) group and CA(-) group (all P〈0.05).The allele frequency and genotype of HPA-1, 2, 3 showed significant differences between the normal group and the diabetes group (all P〈0.05).The allele frequency and genotype of HPA-5 showed significant differences between CA (+) and CA(-) (P〈0.05).Logistic regression analysis showed that age , duration of diabetes, hypertension, smok-ing, overweight, dyslipidemia, genetic polymorphism of HPA-5 were risk factors of CA in patients with type 2 diabetes. Conclusions There is a possible correlation between genetic polymorphism of human platelet antigens HPA -1, 2, 3 and occurrence of type 2 diabetes.Genetic polymorphism of HPA-5 may be the risk factor of type 2 diabetes with CA.
