产科临床药师开展药学服务的实践与体会

Experience of pharmaceutical care by clinical pharmacists in obstetrical department

围产期患者由于体内发生一系列生理变化,会影响药物在体内的动力学改变,且用药后存在胎盘转运和乳汁转运,可影响胎儿组织器官的发育及功能,新生儿对药物的毒副作用也极为敏感,因此面对此特殊人群临床用药需谨慎。本文采用实例探讨临床药师协助医生优化妊娠期、哺乳期的药物治疗方案,对静滴硫酸镁注射液的患者进行重点药学监护,为患者提供用药指导,并及时收集、上报药品不良反应等,保障了患者的用药安全,促进了临床用药的合理性与规范性。

A wide range of physiological changes occur during perinatal period, these changes can significantly affect pharmacokinetics of drugs. The development and function of the tissue and organ in fetus can be infl uenced after use of medicines in perinatal period patients because of placental transport and milk transport. Neonates are particularly sensitive to the toxic and side effect of most kinds of medicines. So the clinical use of drugs should be paid more attention to during perinatal period. In this article, the cases of pharmaceutical care by clinical pharmacists were discussed, such as assisting doctors to optimize the drug therapy regime for pregnant and lactational women, offering pharmaceutical care for patient with intravenous use of magnesium sulfate injection, providing medication education for patients, collecting the adverse drug reactions and so on. These measures ensure the safety of drug utilization in patients and promote the rational drug use in clinic.
