AECOPD患者有创机械通气撤机失败的原因分析 被引量:3

摘要 目的:回顾性分析AECOPD患者有创机械通气撤机失败的危险因素。方法:选择63例机械通气的AECOPD患者进行研究,其中52例患者为撤机成功组;11例患者为撤机失败组。比较两组生理指标、pH、PCO2、浅快呼吸指数、氧和指数、辅助呼吸肌评分、气道分泌物评分、血清白蛋白,脑钠肽等。结果:撤机成功组和失败组在SBT前后的观测指标相对比,心率、平均动脉压、呼吸频率等方面相比没有显著性差异(P>0.05)。在浅快呼吸指数(RSBI)、辅助呼吸肌评分、气道分泌物评分、脑钠肽等上有显著性差异(P<0.05)。结论:呼吸肌疲劳和未减轻的呼吸负荷可能是撤机失败的主要预示指标;同时在撤机时应该高度注意患者的心功能情况。
