双极电凝镊与超声吸引刀在肝叶切除术中应用的对照研究

The Use of Bipolar Coagulation Forceps and Cavitron Ultrasonic Surgical Aspirator in Hepatic Resection-A Randomized Controlled Trial

目的比较超声吸引刀和双极电凝镊在肝叶切除手术中应用的效果。方法选取2010年7月-2012年7月肝叶切除手术患者68例为研究对象。其中采用双极电凝镊行肝叶切除术32例为A组;采用超声吸引刀行肝叶切除术36例为B组。分别比较两种不同方法肝叶切除术患者术前基本参数、术中参数以及术后恢复评估。结果两组患者术前基本参数比较,差异无统计学意义。两组行肝叶切除术失血量和术中输血量比较,差异无统计学意义;手术时间双极电凝镊组短于超声吸引刀组（P〈0.05）。两组患者术后恢复各项指标比较,差异无统计学意义。结论使用双极电凝镊行肝叶切除术止血效果确切,较使用超声吸引刀手术时间短,值得推广。

Objective To compare the effects of the bipolar coagulation forceps and the cavitron ultrasonic surgical aspirator in the hepatic resection.Methods From July 2010 to July 2012,the 68 patients with uniform inclusion and exclusion standards during hepatic resection were selected as the study objectives.The 32 patients used bipolar coagulation forceps hepatectomy were group A and the 36 cases with Cavitron ultrasonic surgical aspirator were group B.Then the 2groups received surgeries from the same surgical team of doctors.The datum were compared between two groups including preoperative basic parameters,operative time,blood loss,transfusion,postoperative alanine aminotransferase,abdominal infection,bile leakage,postoperative intensive care unit time and hospital stays.Results The statistical analysis showed that there were no significant differences in the basic preoperative parameters.Datum suggested that there were no significant differences in intraoperative blood loss and blood transfusion,neither.However,the operation time in the bipolar coagulation forceps group was shorter than the cavitron ultrasonic surgical aspirator group（P〈0.05）.Postoperative recovery datum indicated no statistical difference.Conclusion The use of bipolar coagulation forceps in hepatectomy is worth popularizing with more efficacy and shorter operative time.
