内固定与人工髋关节置换术治疗老年髋部骨折临床疗效比较

Comparison of the clinical effect between internal fixation and artificial hip replacement in treatment of elderly hip fracture patients

目的比较内固定与人工髋关节置换术治疗老年髋部骨折的临床疗效。方法对收治的108例老年髋部骨折患者的临床资料进行回顾性分析,其中48例患者采用内固定手术(内固定组),60例患者采用人工髋关节置换术(置换组),对2组患者的手术一般情况和术后恢复情况进行比较分析。结果与内固定组比较,置换组患者术中出血量明显增加,但负重时间和并发症的发生率明显减少,差异均有统计学意义(P<0.05)。置换组患者术后6、12个月的功能恢复达优率及优良率均明显高于内固定组,差异有统计学意义(P<0.05)。结论人工髋关节置换术治疗老年髋部骨折可缩短卧床时间,患者能早期负重行走,近期疗效也明显优于内固定术。

Objective To compare the clinical effect between internal fixation and artificial hip replacement in treatment of elderly hip fracture patients. Methods The clinical data of 108 cases of elderly patients with hip fracture were analyzed retrospectively. There were 48 patients who were treated by internal fixation surgery （internal fixation group）, and 60 patients who were treated by artificial hip replacement（ displacement group）. The general condition of the surgery and postoperative recovery of the two groups were compared. Results Compared with the internal fixation group,the bleeding amount of patients in the displacement group increased significantly, but the load time and the incidence of complications were obviously less, the differences were statistically significant （ P 〈 O. 05）. The optimal rate and overall fine rate of functional recovery in dis- placement group were significantly higher than those of the internal fixation group six months and twelve months after surgery, the differences were statistically significant （ P 〈 0.05 ）. Conclusion The artificial hip replacement in the treatment on senile hip fracture can effectively shorten the patients＇ bed time, take the postoperative walk as soon as possible, and the recent cura- tive effect is obviously better than the internal fixation.
