选择性阴茎背神经切断术治疗早泄的临床研究

Selective penile dorsal nerve neurotomy for patients with premature ejaculation

目的观察选择性阴茎背神经切断术治疗早泄(PE)的临床疗效。方法选择2009-01—2013-01本院收治的PE患者240例,随机分为两组,观察组120例采用选择性阴茎背神经切断术+口服盐酸舍曲林治疗,对照组120例采用口服盐酸舍曲林治疗。记录患者治疗前后阴道内射精潜伏期(IELT)和CIPE-5评分。结果观察组治疗后IELT值由(0.98±0.32)min提高到(6.15±0.26)min,CIPE-5评分由(9.33±2.59)分提高到(21.26±2.66)分,均明显优于对照组(均P<0.05)。观察组患者的总有效率为85.0%,对照组总有效率为75.8%,观察组优于对照组(P<0.05)。两组均无严重并发症。结论选择性阴茎背神经切断术是治疗PE的一种安全、有效、方便的方法 。

Objective To evaluate the efficacy and safety of selective penile dorsal nerve neurotomy in treatment of pre-mature ejaculation. Methods Two hundred and forty patients with premature ejaculation admitted from January 2009 to Jan-uary 2013 were randomly assigned to two groups： 120 patients were treated with selective penile dorsal nerve neurotomy, in addition to oral sertraline hydrochloride （observation group） and 120 patients were treated with oral sertraline hydrochloride tablets only （control group）. Intravaginal ejaculation latency time （IELT） and Chinese index of sexual function for premature ejac-ulation- 5（CIPE- 5） of all cases were recorded before and after the treatment. Results IELE in observation group was increased from（0.98±0.32） min before treatment to（6.15±0.26） min after treatment and CIPE- 5 scores were increased from （9.33±2.59） to （21.26±2.66）, both indexes were better than those in control group （P〈0.05）. The overal effective rate of observation group was higher than that of control group（85.0%vs 75.8%, P〈0.05）. There were no severe complications in two groups. Conclusion Selective penile dorsal nerve neurotomy is a safe, effective and feasible treatment for patients with premature ejaculation.
