椎间盘镜与椎板开窗治疗单节段腰椎间盘突出症疗效对比分析

Contrast of Discectomy and Laminectomy Treatment of Single Level Lumbar Disc Herniation Analysis

目的：比较椎间盘镜（MED）与椎板开窗髓核摘除术治疗单节段椎间盘突出症（FD）的临床疗效。方法回顾性收集2011年7月～2013年9月我院收治的100例单节段椎间盘突出症患者的临床资料，其中椎间盘镜手术治疗54例，椎板开窗手术46例。重点收集患者手术前后的JOA评分、手术时间、术中出血量、术后住院时间及疼痛等资料，并进行比较。结果两组患者性别、年龄、突出节段、类型等方面差异无统计学意义（P＞0．05）；两组患者手术时间、术中出血量、卧床时间、术后住院时间等方面差异无统计学意义（P＞0．05）；两组手术前JOA评分、腰腿痛VAS评分差异无统计学意义（P＞0．05），术后两组资料腰痛VAS评分差异有统计学意义（P＜0．05），其他均无统计学差异（P＞0．05）；椎间盘镜组疗效优良率为93．7％，椎板开窗组优良率为91．9％，差异无统计学意义（P＞0．05）。结论椎间盘镜治疗单节段腰椎间盘突出具有微创、出血量少、患者康复快、预后较好等优点，值得临床借鉴。

Objective To compare the clinical efficacy of discectomy （ MED） and fenestration discectomy on single segment disc herniation （ FD） treatment.Methods A retrospective clinical data collected from July 2011 to Sep-tember 2013 in our hospital 100 cases of single segment disc herniation ,in which 54 cases of surgical discectomy ,lami-nectomy surgery 46 cases.The collection of patients before and after surgery JOA score ,operative time,blood loss,hospi-tal stay and postoperative pain and other information were focused on and compared .Results There were no significant differences between the two groups of gender ,age,protruding sections,type ect.（P〉0.05）;differences between the two groups of patients operative time ,blood loss,bed time,postoperative hospital stay were not statistically significant （ P〉0. 05）;groups JOA score before surgery and low back pain VAS score showed no significant difference （P〉0.05）,after the two sets of data in addition to back pain VAS score was significantly outside （P〈0.05）,others were not statistically dif-ferent（P〉0.05）;superior efficacy of discectomy group was 93.7%,an excellent fenestration group was 91.9%,the difference was not statistically significant （ P〉0 .05 ） .Conclusion Discectomy for the treatment of lumbar disc single segment with minimally invasive,less blood loss,faster rehabilitation of patients with good prognosis ,etc.is worthy of ref-erence .
