骨髓细胞形态学及免疫表型特征在多发性骨髓瘤诊断中的应用

Function of Bone Marrow Cell Morphology and Immune Phenotype Characteristics in Multiple Myeloma Diagnosis

目的：探讨多发性骨髓瘤细胞细胞形态学检查及骨髓瘤细胞免疫表型特征。方法采用流式细胞仪用多参数直接免疫荧光技术，CD45/CD138设门，对79例多发性骨髓瘤患者骨髓瘤细胞采用直接标记法染色，抗体CD45以PC5标记，CD138以PE标记。抗体CD2，CD7，CD22，CD33，CD38，CD56，CDc 以FITC标记，CD3，CD5，CD10，CD19，CD20，CD25，CD34，HLA-DR以ECD标记，并同时进行骨髓细胞形态学检查。结果形态学检查瘤细胞比例5％～88％，瘤细胞免疫表型相近。 CD138，CD38， CD56抗原阳性表达，其余抗原均为阴性表达。结论 CD45/CD138设门技术可以较准确地设定需分析的多发性骨髓瘤细胞群，与形态学分类瘤细胞的比例接近，可以区别骨髓瘤细胞群和正常瘤细胞群，对诊断骨髓瘤具有明确的诊断意义。

Objective To investigate the morphological examination and immune phenotypic characteristics of multiple myeloma cells.Methods Myeloma cells of 79 cases multiple myeloma patients were directly labeling staining with flow cytometry with multi -parameter direct immunofluorescence technique ,CD45/CD138 gating,the antibody CD45 and CD138 were marked with the PC5 and PE respectively. Antibodies CD2,CD7,CD22,CD33,CD38,CD56,CDc were FITC-labeled and CD3,CD5,CD10,CD19,CD20,CD25,CD34,HLA-DR were marked with ECD ,at the same time bone marrow cell morphological examination was conducted .Results The proportion of tumor cells which was tested by morphological test was 5%~88%,closing to the tumor cell phenotype .The antigen expression of CD138,CD38,CD56 were positive,and the remaining were negative .Conclusion CD45/CD138 gating technology can more accurately set multiple myeloma cell popu-lations needed to analyze , nearly to morphological classification of tumor cells , distinguish myeloma cell populations and normal tumor cell populations and has clear diagnostic significance for diagnosis of myeloma .
