无创正压通气治疗急性低氧性呼吸衰竭的指征 被引量:14

摘要 对急性低氧性呼吸衰竭的治疗,除原发病的处理外,呼吸支持是最为重要的措施。普通氧疗无法提供正压支持,仅适用于病情较轻者;而有创正压通气(IPPV)对通气设备及监护条件要求高,费用昂贵,操作技术复杂,并发症多,资源在国内相对缺乏,不易对呼吸衰竭实施早期干预。无创正压通气(NPPV)具有无创、保留上气道的防御机制、操作简单、低成本、相关并发症较少等优势,在近20年来受到越来越多的关注。NPPV的应用领域也从慢性阻塞性肺疾病急性加重逐渐扩展到各种急性低氧性呼吸衰竭。多项研究显示,对于不同原因导致的急性低氧性呼吸衰竭,NPPV总体上可以改善氧合,减少并发症的发生,降低气管插管率和病死率。
