摘要
大量输血方案(MTP)是指以标准流程的形式指导大出血和创伤性凝血病的治疗,涉及浓缩红细胞、新鲜冰冻血浆、血小板等的输注及重组凝血因子的使用时间、剂量和时间等方面.该过程需要急诊科、外科、检验科、血库等多部门的积极协作.MTP可优化血液制品的输注流程,避免在危急和快速运作情况下发生错误,可减少血液制品的使用总量,提高输血效率,减轻创伤性凝血病的严重程度,改善严重创伤患者的生存率,减少输血相关并发症.现就MTP及其研究进展综述如下.
Massive transfusion protocols (MTP) are well established principles to guide the treatment of massive bleeding and traumatic coagulopathy,which involve the timing,dose and aim of utilization of packed red blood cell (PRBC),fresh frozen plasma (FFP),platelet (PLT) and recombinant factors.These processes require collaboration among different departments,such as emergency department,trauma surgery,laboratory department and blood bank.MTP plays an important role on reducing the amount of blood products used,improving transfusion efficiency,reducing the severity of traumatic coagulopathy,increasing the survival rate of patients and decreasing the transfusion-related complications.Now,MTP and its'research progress are summarized below.
