原发性慢性肾小球疾病与低分子量尿蛋白的关系

摘要 目的:观察原发性慢性肾小球疾病患者尿蛋白分子量与肾小管间质损害及临床指标之间的关系,探讨不同尿蛋白组份在原发性慢性肾小球疾病发病机制中的作用。方法:用聚丙烯酰凝胶电泳(SDS-PAGE)银染法检测50例同步行肾活检诊断明确的原发性慢性肾小球疾病患者尿蛋白分子量。结果:据SDS-PAGE银染法,尿蛋白可分为四组类型:小球性蛋白尿组,生理性蛋白尿组,混合性蛋白尿23 kd组,混合性蛋白尿10 kd组。混合性蛋白尿10 kd组较23 kd组肾小管间质损害严重,肾小管间质损害评分、24 h尿蛋白定量、血清肌酐(Scr)水平均升高(P<0.05)。结论:SDS-PAGE分型10 kd组尿蛋白对判断原发性慢性肾小球疾病肾小管间质损害程度有临床意义,其作为无创性检测手段,可以作为判断肾功能恶化速度及预后的指标。 Objective:To evaluate the role of urinary protein analysis by sodium dodecyl sulfate-polyacrylamide gel elec-trophoresis( SDS-PAGE) in diagonosis and prognosis of primary chronic glomerular diseases, in order to study the mechanism of pro-gression of chronic glomerular diseases. Methods:A total of 50 patients with biopsy proven primary chronic glomerular diseases, men 19, women 31;mean age 27. 5 years were studied. We detect the molecular weight of urinary protein by SDS-PAGE silver staining. Data were processed with the classes of tubulointerstitial lesions and laboratory tests such as 24 h urinary protein quantitation,serum creatinine( Scr) . Results:Scores of tubulointerstitial lesions,twenty-four-hour urine protein quantitation and Scr level were all sig-nificantly higher in 10 kd proteinuria group than 23 kd proteinuria group (P〈0. 05). Conclusion:10 kd proteinuria may have close relationship with the progression of primary chronic glomerular diseases, may be a useful index for the diagonosis and progonosis of primary chronic glomerular diseases.
