DTI与MRS成像在一氧化碳中毒迟发性脑病中的应用研究 被引量:11

摘要 目的探讨核磁共振弥散张量成像(Diffusion tensor imaging,DTI)与磁共振波谱成像(Magnetic resonance spectroscopy,MRS)对一氧化碳中毒迟发性脑病(Delayed neuropsychological sequelae,DNS)的诊断价值。方法随机选取2013年1月-5月间于我科就诊的一氧化碳中毒患者20例,于病情最重时间段和治疗1~2月后分别行MMSE、ADL功能评分和DTI、MRS成像,对数据行统计学分析,观察DTI与MRS结果对DNS的预后评价是否有意义。结果治疗前后MMSE和ADL评分均有明显改善(P<0.05),而DTI、MRS成像结果仅NAA/Cr治疗后有明显改善(P<0.05),其余皆无明显变化(P>0.05)。结论由于本研究所取样本较少,因而某些项目未能呈现有统计学意义的变化,但综合国内外研究及本实验其它显著变化项目可以得出DTI和MRS成像对DNS预后有一定作用,值得继续加以研究。 Objective To investigate the value of DTI and MRS in the diagnosis of delayed neuropsychological sequelae(DNS). Methods 20 carbon monoxide poisoning cases which received by our clinical department were randomly selected during January 2013 to May. MMSE, ADL function evaluation, DTI and MRS were performed to patients in the heaviest condition and 1month^2 months after treatment respectively. The data was analyzed statistically, and the diagnostic value of DTI and MRS to DNS 's prognosis were observed. Results The MMSE and ADL were obviously improved after received treatment(P〈0.05), but the DTI and MRS imaging results are improved only after received NAA/Cr treatment(P〈0.05). Conclusion We have not observed obvious changes in some subjects because of the small sample size. But we can find that DTI and MRS imaging have certain diagnostic significance to the DNS prognosis and have value to be further researched combine with our study and the research at home and abroad.
