摘要 目的探讨MRI平扫和肝脏快速容积成像(LAVA)技术在门静脉海绵样变性(CTPV)的诊断价值及临床意义。方法分析23例经临床证实的CTPV患者的MRI平扫、动态增强LAVA技术横轴位、冠状位及3D-MIP重组图像的表现及特征。结果CTPV的MRI平扫及增强LAVA技术可见门静脉增粗、栓塞或狭窄、部分或完全闭塞,肝门部门静脉周围可见迂曲增多的侧支静脉形成的异常团块状、网状不均匀软组织样等T1短T2信号影明显强化,LAVA冠状位及3D-MIP重组可直观立体显示上述异常病灶。结论 CTPV具有特征性的MRI表现,MRI对CTPV诊断及明确病因具有重要价值。 Objective To discuss the diagnostic value and clinical significance of MRI plain scanning and LAVA technique in portal vein cavernous transformation. Methods To analyze the features and characteristics of MRI plain scanning, dynamic contrast enhanced LAVA transverse view and 3D-MIP recombinant image of 23 cases of CTPV patients who were confirmed by clinic. Results Portal vein enlarging, embolization or stenosis, partly or total occlusion were found by MRI plain scanning and enhanced LAVA technique. Abnormal mass like and reticulodromous uneven soft tissues made by circuital increasing paraesophageal vein were found around hepatic portal vein. T1 was short and T2 was obviously strengthened.These abnormal lesions were directly stereoscopic displayed by LAVA coronal view and 3D-MIP recombinant image. Conclusion The CTPV has characteristic manifestations on MRI, which is valuable in the diagnosis and causation of disease.
